
[摘要] 目的 研究將靜脈血氣分析用于ICU糖尿病酮癥酸中毒患者預(yù)后判斷中的效果。方法 選該院ICU糖尿病酮癥酸中毒60例,在患者發(fā)病的48 h內(nèi)給使用靜脈血氣分析,在15 d后,依據(jù)患者的實際情況,將存活的31例患者作為試驗組,將死亡的29例患者作為對照組,比較相關(guān)指標(biāo)差異。結(jié)果 試驗組的PCO2、HCO3-、BE與對照組相比,均有較為明顯的下降(P<0.05);而試驗組與對照組的PO2與pH值在數(shù)據(jù)上無明顯的變化(P>0.05);兩組患者PO2、PCO2、pH值、HCO3-、BE與APACHEⅡ評分呈正相關(guān)。結(jié)論 在ICU糖尿病酮癥酸中毒患者中,BE水平的高低決定APACHEⅡ評分的高低,有利于對該疾病的診斷。

Evaluation of Venous Blood Gas Analysis in the Prognosis of Patients with Diabetic Ketoacidosis in ICU

[Abstract] Objective To study the effect of venous blood gas analysis on the prognosis of patients with diabetic ketoacidosis in ICU. Methods 60 patients with diabetic ketoacidosis in our hospital were selected, and venous blood gas analysis was used within 48 h of the onset of the disease. After 15 d, according to the actual situation of the patients, 31 patients who survived were used as the test group, and 29 patients died patients were used as the control group, differences in related indicators were compared. Results Compared with the control group, PCO2, HCO3-, and BE in the test group had a significant decrease(P<0.05); while the PO2 and pH values in the test group and the control group had no significant changes in the data(P>0.05); There was a positive correlation between PO2, PCO2, pH, HCO3-, and BE and APACHEⅡ scores in the two groups. Conclusion In patients with diabetic ketoacidosis in ICU, the level of BE determines the level of APACHEⅡ score, which is helpful for the diagnosis of the disease.

糖尿病對于人體的危害,并不是因為高血糖的原因,而是由高血糖所導(dǎo)致的各種急、慢性并發(fā)癥。而常見的急性病并發(fā)癥是糖尿病酮癥酸中毒,而且其尤其多見于ICU糖尿病酮癥酸中毒中,且近幾年,患有該病的人也越來越多。該疾病會導(dǎo)致患者的電解質(zhì)與酸堿度均處于紊亂狀態(tài),其中最為常見的癥狀就是代謝性酸中毒、高血酮、高血糖[1]。而且,該病的治療是近幾年的醫(yī)學(xué)難點,而血氣分析的使用,在極大程度上可以幫助醫(yī)生,對于該病的診斷與治療方案的設(shè)計與實施。因為血氣分析是通過對人體血液中氧、二氧化碳及酸堿平衡等的分析,其可以簡單明了的呈現(xiàn)出人體肺泡功能與其酸堿平衡狀態(tài)[2]。但是,在臨床上,由于糖尿病酮癥酸中毒患者身體的個性化,使血氣分析儀在臨床上的效果不明顯。故選擇的適合的治療手段,對于ICU糖尿病酮癥酸中毒患者預(yù)后判斷就顯得尤為關(guān)鍵。……
