國(guó)內(nèi)梅花針治療斑禿的Meta分析

摘要：目的 ?評(píng)價(jià)梅花針治療斑禿的臨床綜合療效。方法 ?計(jì)算機(jī)全面檢索2010年3月～2020年 3月中國(guó)全文期刊數(shù)據(jù)庫(kù)（CNKI）、萬(wàn)方數(shù)據(jù)庫(kù)和維普中文科技期刊數(shù)據(jù)庫(kù)（VIP）發(fā)表的關(guān)于梅花針治療斑禿的隨機(jī)對(duì)照試驗(yàn)。按照Cochrane系統(tǒng)評(píng)價(jià)，結(jié)合改良Jadad量表，評(píng)價(jià)納入實(shí)驗(yàn)的質(zhì)量，利用RevMan5.3進(jìn)行Meta分析。結(jié)果 ?本研究共納入11篇文獻(xiàn)，總計(jì)809例斑禿患者，僅2篇文獻(xiàn)報(bào)道了斑禿的復(fù)發(fā)情況。Meta分析顯示，與對(duì)照組相比，實(shí)驗(yàn)組梅花針能夠更有效地治療斑禿，差異具有統(tǒng)計(jì)學(xué)意義[RR=1.20>1，95%CI（1.11，1.29），Z=4.76，P<0.00001]，梅花針能夠有效降低斑禿半年復(fù)發(fā)率，差異具有統(tǒng)計(jì)學(xué)意義[RR=0.37<1，95%CI（0.15，0.91），Z=2.17，P<0.03]。結(jié)論 ?梅花針能夠有效治療斑禿，并且使用梅花針治療斑禿能夠有效降低斑禿半年復(fù)發(fā)率，但報(bào)道斑禿復(fù)發(fā)率方面的文獻(xiàn)較少，需要更多高質(zhì)量的研究進(jìn)行驗(yàn)證。

Meta Analysis of Domestic Plum-blossom Needle for Alopecia Areata

Abstract：Objective ?To evaluate the comprehensive clinical efficacy of plum-blossom needle in the treatment of alopecia areata.Methods ?A computer comprehensive search of randomized controlled trials published by China Full-Text Journals Database （CNKI）， Wanfang Database and VIP Chinese Science and Technology Journal Database （VIP） from March 2010 to March 2020 on the treatment of alopecia areata by plum blossom needle. According to the Cochrane system evaluation， combined with the modified Jadad scale， the quality of the included experiments was evaluated， and the Meta analysis was performed using RevMan5.3.Results ?A total of 11 articles were included in this study， with a total of 809 patients with alopecia areata. Only 2 articles reported the recurrence of alopecia areata. Meta analysis showed that compared with the control group， the experimental group plum blossom needle could treat alopecia areata more effectively， the difference was statistically significant [RR=1.20>1，95%CI （1.11， 1.29），Z=4.76， P<0.00001]， Plum-blossom needle could effectively reduce the half-year recurrence rate of alopecia areata，the difference was statistically significant [RR=0.37<1，95%CI（0.15， 0.91），Z=2.17，P<0.03].Conclusion ?Plum-blossom needle could effectively treat alopecia areata， and it could effectively reduce the half-year recurrence rate of alopecia areata. However， there were few literatures on the recurrence rate of alopecia areata， and more high-quality research was needed for verification.

斑禿（alopecia areata）是一種常見(jiàn)的損容性疾病，主要表現(xiàn)為突然發(fā)生的頭發(fā)成片脫落。中醫(yī)稱之為“油風(fēng)”“鬼剃頭”等。中醫(yī)學(xué)認(rèn)為斑禿多因情志不暢，血熱生風(fēng)，血瘀毛竅，使氣機(jī)逆亂，氣血失調(diào)，肝腎陰虧，精血虛少而引起毛發(fā)失榮，突然脫落[1]。中醫(yī)藥治療斑禿的方法主要包括中藥內(nèi)服、中藥外搽、梅花針叩刺、毫針針刺、艾灸、穴位注射等[2]。斑禿治療方法較多，但其治療效果仍存在爭(zhēng)議。《醫(yī)宗金鑒》曰”宜針砭其光亮之處，出紫血，毛發(fā)庶可復(fù)生。“梅花針直接作用于皮損處，具有疏通經(jīng)絡(luò)，調(diào)和氣血的作用，故有利于毛發(fā)的生長(zhǎng)。梅花針是一種有效的治療斑禿的手段，但仍缺乏足夠的循證醫(yī)學(xué)證據(jù)。本研究系統(tǒng)評(píng)價(jià)梅花針治療斑禿的臨床綜合療效，以便臨床能夠合理有效地利用梅花針治療斑禿，現(xiàn)報(bào)道如下。

1資料和方法
