學齡期兒童術前焦慮狀態與術后阿片類需求的相關性分析和預測模型構建

探討學齡期兒童術前焦慮狀態與術后阿片類藥物需求的相關性,并評估據此構建的預測模型的應用價值。選取2018年4月至2020年1月于蕭山區中醫院行扁桃體腺樣體手術的153例患兒為研究對象,術前使用兒童焦慮性情緒障礙篩查量表(screen for child anxiety related emotional disorders,SCARED)、兒童抑郁障礙自評量表(depression self-rating scale for children,DSRSC)進行評估,根據患兒手術后早期阿片類藥物需求情況將其分為需求組(46例)和非需求組(107例),分析影響學齡期兒童手術后早期阿片類藥物需求的相關因素。建立邏輯回歸預測模型并繪制受試者操作特征曲線(receiver operating characteristic curve,ROC曲線)檢驗模型效能。多因素Logistic回歸分析結果顯示,高DSRSC評分、高SCARED評分、女孩均是影響學齡期兒童手術后早期阿片類藥物需求的獨立危險因素(<0.05)。ROC曲線分析顯示,預測模型的敏感度為66.36%,特異性為86.96%。術前高SCARED評分、高DSRSC評分、女孩均是影響學齡期兒童手術后早期阿片類藥物需求的危險因素,聯合以上3個指標建立的預測模型具有較高特異性。

隨著醫療及麻醉技術的不斷發展,學齡期兒童麻醉手術得以廣泛開展,學齡期兒童麻醉手術數量日趨上升。Yang等[1]指出臨床上超過80%的學齡期患兒在麻醉術后經歷不同程度的術后疼痛。學齡期兒童麻醉術后急性疼痛受諸多因素影響[2-3],其中術前焦慮狀態可能是一個重要因素。因此,本研究通過建立預測模型的方式探索學齡期兒童麻醉手術后早期阿片類藥物需求的相關因素,現將結果報道如下。
選取2018年4月至2020年1月于蕭山區中醫院靜脈–吸入復合全身麻醉行扁桃體腺樣體手術的153例患兒為研究對象。納入標準:①年齡7~12歲;②行扁桃體及腺樣體等離子切除術;③認知功能正常,能夠自我報告術后疼痛感受;④臨床資料完整。排除標準:①既往存在精神疾病史及認知功能障礙者;②存在聽力、語言障礙,無法有效溝通;③合并心、肝、肺、腎功能障礙及腦部器質性疾病者;④患有慢性疼痛性疾病或術前服用阿片類藥物者。本研究經蕭山區中醫院倫理委員會批準(倫理審批號:2020-倫審-52),患兒家屬均知情同意并簽署同意書。
1.2.1 臨床資料收集 收集所有患兒的性別、年齡、體質量指數(body mass index,BMI)、手術史、麻醉等級。使用兒童焦慮性情緒障礙篩查量表(screen for child anxiety related emotional disorders,SCARED)、兒童抑郁障礙自評量表(depression self-rating scale for children,DSRSC)評定患兒術前心理狀態;SCARED共41個條目,DSRSC共18個條目,每條目0~2分,評分越低心理狀況越好。記錄兩組患兒的手術時間、術畢收縮壓和舒張壓水平,測量術畢體溫。清醒后即刻使用視覺模擬評分法(visual analogue scale,VAS)對患兒的疼痛情況進行評價,將標尺兩端文字說明換為小兒面孔,一端笑容的小兒面孔表示無痛(0分),另一端痛苦面容的圖形表示重度疼痛(10分);0~2分代表無痛,3~4分代表輕度疼痛,5~7分代表中度疼痛,8~10分代表重度疼痛。
1.2.2 小兒麻醉手術后早期鎮痛原則 術后根據三階梯鎮痛原則,VAS評分為輕度疼痛予對乙酰氨基酚30mg/kg塞肛鎮痛;中度疼痛予曲馬多50~100mg口服加對乙酰氨基酚30mg/kg塞肛鎮痛;重度疼痛予嗎啡5~10mg靜脈鎮痛。術后重度疼痛使用嗎啡鎮痛的患兒定義為有阿片類藥物需求。
1.2.3 預測模型的構建 本研究限于樣本量,不按比例單獨劃分訓練集和測試集,而采用十折交叉驗證方法建立模型,即每次將數據隨機分為10份,其中9份作為訓練集,1份作為測試集,該過程重復10次,以防止“過擬合”現象的出現。根據多因素Logistic回歸分析得出影響學齡期兒童麻醉手術后早期阿片類藥物需求的獨立相關因素,通過建立邏輯回歸預測模型,使用medcalc軟件計算預測模型數據,帶入受試者操作特征曲線(receiver operating characteristic curve,ROC曲線)進行效能分析。

153例患兒中術后需要阿片類藥物輔助鎮痛46例,發生率30.07%,據此將患兒分為需求組(46例)和非需求組(107例),兩組患兒的SCARED評分、DSRSC評分、性別、手術時間比較,差異均有統計學意義(<0.05),見表1。

多因素Logistic回歸分析結果顯示,高DSRSC評分、高SCARED評分、女孩均是影響學齡期兒童手術后早期阿片類藥物需求的獨立危險因素(<0.05),見表2。
以Logistic回歸分析結果中最小值0.316為基數,其他變量值與其相除得出各變量的賦值分數,最終按照賦值公式Y=10×X1+10×X2+10×X3計算得分,變量賦值分數見表3;以該得分為檢驗變量X,以患兒是否需要阿片類藥物為因變量,繪制ROC曲線,結果顯示預測模型的曲線下面積為0.813(95%:0.743~0.872),敏感度為66.36%,特異性為86.96%,見圖1。

盡管學齡期兒童麻醉手術及術后護理取得顯著進步,但絕大部分手術患兒常經歷術后急性疼痛[4-6]。阿片類藥物是圍手術期疼痛治療的基石,但學齡期兒童手術后常規使用阿片類藥物可增加患兒胃腸道不適、呼吸抑制等風險[7-9]。Donohoe等[10]指出麻醉術后18%~33%的學齡期兒童需使用阿片類藥物鎮痛,但術后患兒常規使用阿片類藥物是不符合臨床術后鎮痛藥物使用原則的。如果術前能夠通過有效的方式識別學齡期兒童手術后早期阿片類藥物需求的相關因素,對指導阿片類藥物干預具有重要意義。
本研究結果顯示,30.07%的學齡期扁桃體腺樣體手術患兒術后對阿片類藥物有需求,與Nafiu等[11]報道結果一致。術前高DSRSC評分和高SCARED評分均是影響學齡期兒童手術后早期阿片類藥物需求的獨立危險因素,究其原因:術前高SCARED評分表明患兒的焦慮情緒明顯,術前焦慮的患兒常表現為睡眠質量差、情緒低落等。Shin等[12]研究發現術前焦慮或抑郁情緒是使用鎮痛藥物的危險因素。Pagé等[13]研究表明,焦慮可增加髖關節手術患者術后疼痛程度。Sibanda等[14]研究指出,針對術前存在焦慮、抑郁情緒的患者實施心理干預,可有效減少患者術后疼痛的發生。
本研究結果顯示女孩也是影響學齡期兒童手術后早期阿片類藥物需求的危險因素。腦部正電子發射斷層掃描成像顯示,男性在疼痛時大腦主管認知功能的部位活動較強烈,大腦的“思考部位”對疼痛作出反應,通過集中感官注意力可減輕其疼痛感受[15-17];而女性出現疼痛時,大腦主管情感反應的邊緣區域活動較強,而情感通過改變腦內神經遞質和代謝使疼痛更敏感[18-19]。
另外,本研究結果顯示預測模型的曲線下面積為0.813,截斷值為53,敏感度為66.36%,特異性為86.96%。通過建立這種評分式的預測模型,能夠有效幫助臨床護士和麻醉醫生簡單高效地識別術后早期對阿片類藥物有需求的人群。由于本研究樣本量較少且部分患兒進行評分時受到主觀因素的影響,最終結果可能產生偏倚,后期還需增大樣本量,運用更加科學有效的方式來驗證該結論。
綜上,女孩、高SCARED評分和高DSRSC評分均是學齡期兒童扁桃體腺樣體手術后早期阿片類藥物需求增加的危險因素。聯合以上3個指標建立的預測模型具有較高的特異性,對術后阿片類藥物需求有一定預測作用。

Correlation analysis and prediction model construction of preoperative anxiety state and postoperative opioid demand in school-age children

To explore the correlation between preoperative anxiety and postoperative opioid demand of school-age children, and to evaluate the application value of the prediction model.A total of 153 children who underwent tonsillectomy in Xiaoshan District Hospital of Traditional Chinese Medicine from April 2018 to January 2020 were selected as the research object. Preoperative evaluation was performed using screen for child anxiety related emotional disorders (SCARED), depression self-rating scale for children (DSRSC). Children were divided into demand group (46 cases) and non-demand group (107 cases) according to the demand of opioid drugs in early postoperative period, and the related factors affecting the demand of opioid drugs in early postoperative period of school-age children were analyzed. The Logistic regression prediction model was established and the receiver operating characteristic (ROC) curve was drawn to test the model efficiency.The results of multivariate Logistic regression analysis showed that high DSRSC score, high SCARED score and girls were all independent risk factors affecting the early demand for opioids in school-age children after operation (<0.05). ROC curve analysis showed that the sensitivity and specificity of the prediction model were 66.36% and 86.96%.High SCARED score, high DSRSC score and girls are all risk factors that affect the demand for early opioid drugs in school-age children after operation, and the prediction model established by combining the above three indexes has high specificity.
