內(nèi)鏡下套扎聯(lián)合普萘洛爾治療食管靜脈曲張破裂出血近期療效觀察

[摘要] 目的 研究?jī)?nèi)鏡下套扎聯(lián)合普萘洛爾治療食管靜脈曲張破裂出血的臨床效果。 方法 選取我院2009年1月～2012年6月就診的100例食管靜脈曲張破裂的患者，設(shè)為觀察組，采用內(nèi)鏡下套扎聯(lián)合普萘洛爾的方法，選取同期就診的100例食管靜脈曲張破裂的患者，設(shè)為對(duì)照組，采用內(nèi)鏡下套扎的治療方法。比較兩組的臨床效果。 結(jié)果 內(nèi)鏡下套扎聯(lián)合普萘洛爾治療食管靜脈曲張破裂出血的有效止血率高，再出血率低，能夠減輕食管靜脈曲張嚴(yán)重程度。 結(jié)論 內(nèi)鏡下套扎聯(lián)合普萘洛爾是一種安全有效的治療食管靜脈曲張破裂的方法，值得在臨床上大規(guī)模推廣。

Recent curative effect observation of loop ligature under Endoscope combined with propranolol in treatment of esophageal varices bleeding

[Abstract] Objective To study the clinical effect of loop ligature under Endoscope combined with propranolol in treatment of esophageal varices bleeding. Methods Selected 100 patients with esophageal varices in our hospital from January 2009 to June 2012 were established as the observation group and were treated by loop ligature under Endoscope combined with propranolol，in the corresponding time period 100 patients with esophageal varices were selected as the control group，and were treated by loop ligature under Endoscope.The clinical effect of two groups were compared. Results The effective hemostasis rate of observation group was higher，rebleeding rate was lower，could reduce the severity of esophageal varices. Conclusion Loop ligature under Endoscope combined with propranolol is a safe and effective method for the treatment of esophageal varices，worthy of a large-scale promotion in clinic.

食管靜脈曲張破裂出血是肝硬化失代償期嚴(yán)重并發(fā)癥之一，易造成大出血死亡[1]。有文獻(xiàn)報(bào)道，其發(fā)病率為40%，病死率為25%～45%[2]。臨床上常見(jiàn)的止血手段是內(nèi)鏡下套扎術(shù)。單獨(dú)使用內(nèi)鏡下套扎術(shù)治療食管靜脈曲張破裂出血容易發(fā)生結(jié)扎不徹底、門(mén)靜脈壓力增高等情況，療效不能保持穩(wěn)定長(zhǎng)久。國(guó)內(nèi)外有文獻(xiàn)報(bào)道，使用內(nèi)鏡下套扎聯(lián)合藥物治療食管靜脈破裂出血的病例[2]。本研究選取本院就診的100例食管靜脈曲張破裂的患者，采取內(nèi)鏡下套扎聯(lián)合β-受體阻滯劑治療食管靜脈破裂出血，觀察其近期臨床效果，現(xiàn)將結(jié)果報(bào)道如下。

1 資料與方法

1.1 一般資料

選取我院2009年1月～2012年6月就診的100例食管靜脈曲張破裂的患者，其中男75例，女25例，年齡28歲～56歲，平均（39.2±10.1）歲。……
