臨床藥師參與心血管內科查房效果分析

[摘要] 目的 探討臨床藥師參與心血管內科查房效果分析。 方法 對臨床藥師參與心血管內科查房記錄的藥例進行統計學分析，并結合相應典型病例說明。 結果 2011年9～12月共查房記錄327例藥例，其中制定個體化用藥方案157份（48.0%），糾正用藥錯誤66份（20.2%），縮短療程及療效提高38份（11.6%），住院費用減少24份（7.3%）。 結論 臨床藥師參與心血管內科查房可以促進藥物合理利用、降低錯誤用藥的發生率，提高治療水平，值得在臨床上推廣應用。

Effect analysis of clinical pharmacists participating in the clinical rounds of cardiovascular internal medicine

[Abstract] Objective To investigate the effect of clinical pharmacists participating in the clinical rounds of cardiovascular internal medicine. Methods Drug cases of clinical pharmacists participating in the clinical rounds of cardiovascular internal medicine were statistically analyzed，and explained combining with corresponding typical case. Results 327 cases of drug cases were recorded from September to December 2011，including 157 cases（48.0%）of setting individualized regimen，66 cases（20.2%）of correcting medication errors，38 cases（11.6%）of shortening period of treatment and improving curative effect，24 cases（7.3%）of reducing hospital costs. Conclusion Clinical pharmacists participating in the cardiovascular internal medicine ward round can promote reasonable drug use，reduce the incidence of wrong medicine，improve the level of treatment，worthy of popularizing in clinical application.

心血管疾病，又稱為循環系統疾病，是一系列涉及循環系統的疾病，患者往往病情危重，用藥復雜。作為一名心血管專業臨床藥師，不僅要參與心血管內科查房、會診、病例討論及對用藥醫囑進行審核等醫療活動全過程，同時也要對患者用藥進行合理化指導，按照醫囑或處方，對即將出院的帶藥種類復雜和初診的患者，進行合理用藥教育及安全用藥指導，以促進患者正確用藥，并提高其用藥依從性[1]。本研究總結了臨床藥師參與心血管內科查房的實踐及體會，報道如下。

1 資料與方法

1.1 一般資料

選取專職臨床藥師2名，深入我院心內科隨臨床醫師查房，針對醫師合理用藥方面存在的問題提出合理建議。選取我院2011年9～12月臨床合理用藥干預及追蹤結果記錄，按照干預項目及療效進行統計、分析。
