乳腺癌手術體位的改進在臨床應用中的研究

摘要：目的 對比研究乳腺癌手術體位的改進在治療乳腺癌改良根治術中的療效。方法 改良組：采用改進后的手術體位45例；傳統組：采用傳統手術體位42例。比較兩組手術時間、淋巴結清掃例數、術后患側肢體近期不適感發生的例數、術后患側肢體浮腫的例數指標。結果 改良組與傳統組術中手術時間、淋巴結清掃例數、術后患側肢體近期不適感發生的例數均有顯著性差異（P<0.01）。結論 改良后的手術體位在乳腺癌改良根治術中明顯優于傳統手術體位，值得臨床推廣。

Abstract：Objective comparing the improvement of breast cancer surgery position in the treatment of breast cancer modified radical curative effect.Methods Improved group：45 cases using the improved operation position；Traditional groups：42 cases with traditional surgery positions.The lymph node compare two groups of operation time，the number of cases， postoperative side body discomfort cases happened recently，postoperative cases of lateral limb swelling index.Results Improved group and traditional group in the lymph node，the number of cases operation time and postoperative side body discomfort recent cases all have significant difference （P<0.01）.Conclusion The modified surgical posture is superior to the traditional surgery in breast cancer modified radical positions，worth clinical promotion.

乳腺癌是全世界婦女第一位惡性腫瘤，并且發病率每年約以2%的速度遞增，全世界每年約有 120萬婦女發生乳腺癌，有50萬婦女死于乳腺癌。近年來新病例數以每年3%～4%的速度遞增，超出全球1%～2%[1-2]。在北京、上海等大城市，乳腺癌已經成為婦女第一位惡性腫瘤[3-4]。乳腺癌嚴重威脅著女性的健康。近些年，在我們醫院因乳腺癌住院的患者數量不斷增加，接受乳腺癌手術治療的病例數也在不斷增多。乳腺癌的治療以根治性手術治療為基礎，同時配合化療、放療、內分泌治療及靶向治療的綜合治療方案[5]。目前臨床上常用的乳腺癌手術治療方式有：標準乳腺癌根治術、改良乳腺癌根治術、乳腺癌擴大根治術、單純乳房切除術、乳房部分切除術及乳腺癌保乳手術，乳腺癌改良根治加乳房一期成形術。無論哪種手術術式，手術術野的顯露都尤為重要。
