運用循證藥學(xué)原理實例探討藥師價值

摘要：目的 探討藥師在新的藥學(xué)模式下如何實現(xiàn)自我價值。方法 通過藥學(xué)服務(wù)的實例，探討改變藥師現(xiàn)行的工作方式的方法，建議處方管理辦法增加藥學(xué)服務(wù)記錄，處方跟蹤記錄等內(nèi)容。使醫(yī)院藥學(xué)模式向以患者為中心的藥學(xué)服務(wù)模式轉(zhuǎn)換。運用循證藥學(xué)原理藥師改變了自我工作方式，使藥師從繁重的處方調(diào)配中解放出來，藥師的工作重心轉(zhuǎn)向處方設(shè)計，藥學(xué)服務(wù)，效應(yīng)跟蹤。結(jié)果 我院藥師工作轉(zhuǎn)向提高了我院藥學(xué)服務(wù)質(zhì)量。結(jié)論 藥師只有從狹隘的窗口中走出，積極主動參與患者的藥學(xué)服務(wù)才能實現(xiàn)自我價值。合理用藥需要藥師改變現(xiàn)行工作方式。

Discussion for the role of pharmacist with examples evidence-based pharmacy principle

Abstract：Objective Try to find ways how the pharmacists realize own role in new pharmacy practice. Methods With examples of medical care，suggestions for prescription legislation，such as medical care record，pursue prescription record. transform present pharmacy to the one that patients are center. Free pharmacist from their heavy filling prescription. Pharmacist change their boring job to prescription design，medical care，pursue prescription which will benefit rational use of medicine.Results Transformation improve medical care in our hospital .Conclusion This is the only way that pharmacist realize virture，as well as benefit rational use of medicine. Walk out their narrow service of window and join medical care positively.

循證藥學(xué)指藥師在藥學(xué)實踐中需要依據(jù)其專業(yè)知識和經(jīng)驗，并綜合考慮當(dāng)前可獲得的最佳研究證據(jù)和患者的需求與偏好制定決策。廣義的循證藥學(xué)是貫穿藥學(xué)科學(xué)研究、實踐與管理。國際藥學(xué)聯(lián)合會提倡傳統(tǒng)和現(xiàn)代醫(yī)藥都要走循證藥學(xué)的道路，從合理用藥角度出發(fā)，用科學(xué)用藥逐步代替經(jīng)驗用藥，最大限度減少藥品使用的盲目性。

1 藥師改變傳統(tǒng)藥學(xué)實踐的嘗試

傳統(tǒng)藥學(xué)實踐模式以藥品為中心，以藥品提供、調(diào)劑為主要職責(zé)，而循證藥學(xué)實踐強調(diào)以患者為中心，關(guān)注患者從藥品選擇，處方調(diào)劑到使用的全過程，因此是以患者為中心的全程藥學(xué)服務(wù)。國際藥學(xué)聯(lián)合會指出：藥師的作用從傳統(tǒng)配藥發(fā)放藥品轉(zhuǎn)移到藥學(xué)服務(wù)及合理用藥信息的提供者。……
