莪術(shù)油葡萄糖注射液致過敏性休克臨床觀察

摘要：目的 探討莪術(shù)油葡萄糖注射液的不良反應(yīng)的特點(diǎn)。方法 2例患者（例1 8歲，例2 12歲），靜脈滴注莪術(shù)油葡萄糖注射液5min出現(xiàn)過敏性休克。例1患者呼吸困難，大汗淋漓，面色蒼白，鼻翼煽動，口唇發(fā)紺，情緒煩躁。停用莪術(shù)油葡萄糖注射液，給予靜脈推注地塞米松10mg，肌注異丙嗪25mg 10min后癥狀緩解；例2患者口干，心慌，氣短，冷汗，面色蒼白，皮膚瘙癢繼而出現(xiàn)，全身發(fā)冷，神志恍惚。給予對癥處理后好轉(zhuǎn)。結(jié)果 莪術(shù)油葡萄糖注射液的不良反應(yīng)多在1～5min內(nèi)發(fā)生，主要是過敏反應(yīng)，與用藥劑量無關(guān)。結(jié)論 預(yù)防過敏反應(yīng)，能夠明顯減少莪術(shù)油葡萄糖注射液的不良反應(yīng)。

Clinical Observation of Aallergic Shock Induced by Zedoary Turmeric Oil and Glucose Injection

Abstract：Objective To explore the characteristics of adverse reactions Zedoary Turmeric Oil and Glucose Injection. Methods 2 patients （patients 1 8 years of age， 2 12 years）， intravenous drip Zedoary Turmeric Oil and Glucose Injection 5min anaphylactic shock. In 1 patients with difficulty in breathing， sweating， pale， flaring nostrils， lips cyanosis， irritable mood. Disable Zedoary Turmeric Oil and Glucose Injection， given an intravenous bolus injection of dexamethasone 10 mg， 25 mg intramuscular promethazine 10min after the remission of symptoms； 2 patients with dry mouth， palpitation， shortness of breath， sweat， complexion is cadaverous， skin itching and， the whole body is algid， trance. Give a symptomatic treatment. Results The adverse reaction of Zedoary Turmeric Oil and Glucose Injection multi occurred in 1--5min， mostly allergic reactions， independent of dosage. Conclusion To prevent allergic reaction， can significantly reduce adverse reaction Zedoary Turmeric Oil and Glucose Injection.

當(dāng)前中藥注射劑作為抗病毒和抗生素的替代品，因其免做皮試，為臨床治療提供了方便。莪術(shù)油葡萄糖注射液（ZNG）可以與多種抗生素同瓶配伍用藥，但隨之而來的不良反應(yīng)也相應(yīng)增多，所以在使用過程中應(yīng)引起高度重視。本文通過下列2例莪術(shù)油葡萄糖注射液致過敏性休克報道，為臨床合理用藥提供參考。

莪術(shù)油葡萄糖注射液是一種抗菌、抗病毒的中藥制劑，系莪術(shù)油、葡萄糖與助溶劑配置的滅菌水溶液。莪術(shù)油中的莪術(shù)醇對呼吸道合包病毒（RSV）有直接抑制作用，對流感病毒A1和A3型有直接的滅活作用。該藥其質(zhì)量標(biāo)準(zhǔn)已收人中國藥典1995版第二部。該藥最初只作為抗腫瘤藥用于臨床，現(xiàn)在臨床上廣泛用于上呼吸道感染、流行性感冒、病毒性肺炎等。……
