氨基酮戊酸光動力療法治療皮膚癌前病變及淺表皮膚癌的臨床療效探討

[摘要] 目的 探討氨基酮戊酸光動力療法治療皮膚癌前病變及淺表皮膚癌的臨床療效。方法 選取該院皮膚科2011年3月—2014年6月間就診的皮膚癌前病變及皮膚淺表癌患者60例，對其采用氨基酮戊酸光動力療法治療，然后觀察其臨床療效及不良反應。結果 日光性角化及疣狀癌的總有效率為100%，基底細胞癌的總有效率為95.2%，鱗癌的總有效率為85.7%，鮑溫病的總有效率為14.3%，pa的總有效率為88.9%，不同類型疾病之間的療效相比，差異無統計學意義（P>0.05）。經過半年的隨訪，發現患者容易出現復發現象，疣狀癌的復發率為25%，日光性角化疾病的復發率為20%，鱗癌的復發率為14.3%，鮑溫病的復發率為14.3%，基底細胞癌的復發率為14.2%，Paget病的復發率為11.1%，通過對各疾病之間復發率進行統計分析可知，差異無統計學意義（P>0.05），且不良反應較輕。結論 應用氨基酮戊酸光動力療法治療皮膚癌前病變及淺表皮膚癌，臨床療效較好、無痛苦、患者較為滿意，具有臨床應用推廣價值。

[Abstact] Objective Discuss the clinical curative effect observation on ALA-PDT in the treatment of skin lesions and superficial skin cancer before.Methods Select superficial lesion before visits between skin cancer and skin cancer patients with 60 cases of our hospital dermatological department in March 2011 to June 2014，and observe the clinical curative effect and adverse reactions. Results The solar sex Angle and verrucous carcinoma of the total effective rate was 100%，the total effective rate was 95.2% of basal cell carcinoma， squamous cell carcinoma of the total effective rate was 85.7%， bao warm disease the total effective rate was 14.3%， pa of the total effective rate was 88.9%， the curative effect between different types of diseases compared with no statistically significant difference （P>0.05）. After six months of follow-up， found patients prone to recurrent phenomenon， verrucous carcinoma recurrence rate was 25%， the Angle of solar sex disease recurrence rate was 20%， and squamous cell carcinoma recurrence rate was 14.3%， the bao of warm disease recurrence rate was 14.3%， basal cell carcinoma recurrence rate was 14.2%， Paget's disease recurrence rate was 11.1%， through the study of the statistical analysis of various disease recurrence rates between， there was no statistically significant difference （P>0.05）， and the adverse reaction is lighter.Conclusion ALA-PDT has satisfactory effects in the treatment of skin lesions and superficial skin cancer before，The clinical curative effect is better， patients have no more pain and been satisfacted，it has the clinical application promotion value.

近年來，皮膚癌前病變及皮膚淺表癌[1]的發病率呈增高趨勢，給人們的心里帶來了沉重的負擔。臨床上目前用于皮膚癌治療的方法主要有手術、冷凍及局部外用藥等，然而由于手術治療易引起組織缺損且容易留下瘢痕，又因局部外用藥物及冷凍治療方法存在局限性，較易復發，因此臨床應用受限。隨著醫學技術的不斷進步，光動力療法[2]因其腫瘤組織創傷小、選擇性好且效果較佳而被廣泛用于臨床。該研究隨……
