選擇性背神經(jīng)阻斷術治療早泄的初步研究

[摘要] 目的 研究選擇性背神經(jīng)阻斷術治療男性早泄的有效性和安全性。 方法 方便選取2011年12月—2015年6月該院收治的64例患者行選擇性陰莖背神經(jīng)阻斷術，比較手術前后陰道內(nèi)射精潛伏期（intravaginal ejaculatory latency time，IELT），國際勃起功能指數(shù)-5（index of international erectile function-5， IIEF-5）變化，并評估術后并發(fā)癥情況。 結果 64例早泄患者平均年齡（29.7±6.6）歲，手術時間平均（49.5±12.4）min，平均切斷背神經(jīng)分支（4.2±1.1）支。術前平均IELT評分（1.3±0.5）min，術后3個月平均IELT評分（3.6±1.6）min，術后6個月平均IELT評分（3.6±1.4）min，均有明顯改善，且術后IIEF-5評分無顯著變化，對患者勃起功能無明顯影響，并發(fā)癥發(fā)生率低。 結論 選擇性陰莖背神經(jīng)阻斷術在短期內(nèi)可明顯延長IELT，手術安全性高，對于患者勃起功能無明顯影響，并發(fā)癥發(fā)生率低，對于男性早泄的治療可取得明顯效果。

[Abstract] Objective To investigate the efficacy and safety of selective resection of dorsal nerves of penis for premature ejaculation. Methods Convenient selection since December 2011 to June 2015 total of 64 premature ejaculation patients accepted selective resection of dorsal nerves of penis， we compared the changes of intravaginal ejaculatory latency time（IELT）， index of international erectile function-5（IIEF-5） before and after operation， followed up and evaluated the complication. Results The mean age of patients was（29.7±6.6） years old， mean operation time was （49.5±12.4）minutes， mean removed branches of dorsal nerve of penis were （4.2±1.1）. The mean IELT was （1.3±0.5）minutes before operation， after 3 and 6 months follow-up， it reached up to （3.6±1.6）minutes and （3.6±1.4）minutes， respectively， which were significantly improved. The IIEL-5 score had no evident change after operation， which meant that erectile function was not influenced significantly. Meantime， the postoperative complication rate was low. Conclusion Selective resection of the dorsal nerves of penis can increase IELT significantly in a short term， with a high safety and low complication rate， whereas erectile function is not affected. The results imply that selective resection of the dorsal nerves of penis may be an effective method for the treatment of premature ejaculation for some patients.

早泄是男性常見疾病之一。流行病學調(diào)查顯示，在成年男性中的的發(fā)病率35%～50%[1]。時至今日，學術界并沒有統(tǒng)一的早泄定義，國際性醫(yī)學會（International Society for Sexual Medicine， ISSM）將其定義為射精往往或總是在插入陰道之前或插入之后1 min之內(nèi)發(fā)生；對大多數(shù)或每次插入陰道后，沒有控制延長射精的能力；有消極的后果，例如煩惱、痛苦、沮喪或/和逃避親密接觸等[2]。在早泄的治療中，目前仍缺乏有效的藥物經(jīng)美國食品和藥物管理局（Food and Drug Administration， FDA）認證批準。而近期研究發(fā)現(xiàn)，陰莖背神經(jīng)興奮性過高與早泄的發(fā)生關系密切。自2011年12月—2015年6月方便選擇該院收治的64例患者采用選擇性陰莖背神經(jīng)阻斷術治療早泄，取得了令人滿意的療效，現(xiàn)報道如下。……
