肺外小細胞癌的臨床病理特征及綜合治療策略

[摘要] 目的 分析肺外小細胞癌（extrapulmonary small cell carcinoma，EPSCC）的臨床病理特征及不同治療模式對預后的影響。 方法 方便選取并回顧性分析2012年8月—2015年12月福建省立醫院收治20例肺外小細胞癌患者的臨床病理特征及治療方法、預后分析。 結果 共20例EPSCC，男15例，女5例，平均年齡61.1歲。I期2例，II期4例；III期10例；IV期4例；原發灶位置：食管12例，縱膈2例，胃食管交界處、閉孔、輸尿管、子宮、卵巢各1例，原發病灶不明1例。采用化療或手術的患者中位OS均為6個月，均無2年以上生存的病例，采用綜合治療的患者中位OS 15.2個月，1年生存率50%，2年生存率12.5%年以上，對癥支持治療的患者中位OS 2.1個月。結論 肺外小細胞癌發病率低，目前尚無標準的治療方法，采用綜合治療模式優于單一治療方式。

[Abstract] Objective Extra-pulmonary small cell carcinoma （EPSCC） is a rare cause of malignancy. The purpose of this study is to clinicopathological characteristics of patients with EPSCC， and to analyze different therapeutic methods. Methods Convenient selection and retrospective review of all patients with EPSCC between August 2012 and December 2015 was conducted.Patients included for analysis had pathologic diagnosis of EPSCC， poorly differentiated tumors， and negative chest imaging at diagnosis. Results 20 patients were included in the analysis. Male 15 patients，female 15 patients；The primary disease sites were： carcinoma of unknown primary 1 patients， esophagus 12 patients， mediastina 2 patients ，and gastroesophageal junction， obturator， ureters， uterus and ovary 1 patients， respectly；Overall survival：The median overall survival （OS） was 6 months for surgery or chemotherapy， 12.5 months for integrated therapy，2.1 months for support therapy. Conclusion EPSCC is a rare cause of malignancy.， there is no standard treatment method， the comprehensive treatment mode is better than the single treatment.

小細胞肺癌（small cell lung carcinoma，SCLC）多發生于肺組織內，而肺外小細胞癌（extrapulmonary small cell carcinoma，EPSCC）是指發生于人體肺以外其他器官的小細胞癌，是十分罕見的一類腫瘤。EPSCC是一種分化程度低、具有高度侵襲轉移能力的惡性腫瘤。因其發病率低，目前僅有個別中心回顧性病例報道[1-3]，大多數已知的小細胞癌的流行病學及診斷治療是來自小細胞肺癌（small cell lung carcinoma，SCLC）。事實上，越來越多的研究報道EPSCC既不同于SCLC，也與其發生部位的常見惡性腫瘤不同，而有其獨特的臨床病理特征，遺憾的是，因臨床不多見，常就診于各個科室，得不到合理的治療。該文采用回顧性分析該院2012年8月—2015年12月福建省立醫院收治20例肺外小細胞癌患者的臨床病理特征及治療方法、預后分析，探討EPSCC的臨床病理特征及綜合治療策略，現報道如下。……
