
[摘要] 目的 探究碎裂QRS波在急性心肌梗死患者診斷及預后中的應用價值。方法 整群選取2014年11月—2016年1月在該院心內科就診的248例冠心病患者。按是否為AMI分為兩組各124例。比較不同心電圖指標在AMI中的診斷價值,并記錄不同心電圖病理波形者發生的并發癥及心源性死亡的發生率。結果fQRS波合并病理性Q波敏感度為36.29%,特異性為96.77%,陽性預測值為91.83%,高于單純fQRS波、單純病理性Q波。此外,有fQRS波的患者77例,嚴重并發癥發生率為53.25%,心功能不全發生率為49.35%,死亡發生率為19.48%。與無fQRS波患者存在顯著性差異。結論 fQRS波用于AMI早期診斷可靠性高,靈敏度好、特異性強,具有臨床推廣價值。

[Abstract] Objective To explore the fragmented QRS wave in acute myocardial infarction diagnosis and prognosis of patients with application value. Methods Group selection November 2014 to January 2016 in the Department of Cardiology in our hospital for treatment of 248 cases of coronary heart disease patients. According to whether AMI were divided into two groups, each group of 124 cases. Diagnostic value of different electrocardiographic criteria in patients with acute myocardial infarction (AMI) and record the incidence of different pathologic ECG waveform, complications and cardiac death. Results The fQRS with pathological Q wave sensitivity was 36.29% and specificity was 96.77%, the positive predictive value was 91.83%, higher than that of pure fQRS And simple pathological Q wave. In addition, fQRS of 77 patients with, serious complications occur rate is 53.25%, heart function, the incidence of 49.35%, the death rate for 19.48%. and no fQRS patients, there was significant difference. Conclusion The fQRS waves are used for the early diagnosis of AMI reliability high, excellent sensitivity, strong specificity, with clinical value.

急性心肌梗死(AMI)是臨床上常見的嚴重疾病,目前通過觀察心電圖ST段及Q波相關變化診斷AMI是一種較為常用的手段。但近年來隨著強效溶栓劑及冠脈介入治療的發展,患者出現Q波心肌梗死逐漸減少,心電圖ST段及Q波作為觀察指標受到一定限制[1]。該次研究整群選取2014年11月—2016年1月在該院心內科就診的248例冠心病患者,采用碎裂QRS波(fQRS)對AMI患者進行診斷,取得較好療效,現報道如下。
1 資料與方法
1.1 一般資料
整群選取在該院心內科就診的248例冠心病患者。對照組:非AMI冠心病,124例,男69例,女55例,年齡47~78歲,平均年齡(59.1±4.88)歲。研究組:AMI患者,124例,男68例,女56例,年齡51~78歲,平均年齡(60.2±4.91)歲。兩組患者在性別、年齡等方面差異無統計學意義,P<0.05。
1.2 心電圖診斷
采用力康PC-3000多參數監護儀對兩組患者進行心電圖檢查,包括單純fQRS波、單純病理性Q波、fQRS波合并病理性Q波。……
