加味理沖湯治療非酒精性脂肪肝臨床研究

[摘要] 目的 研究證實加味理沖湯治療非酒精性脂肪肝的臨床療效。方法 采用隨機對照的試驗方法，整群納入2014年3月—2016年3月該院收治的102例非酒精性脂肪肝患者，其中治療組51例，對照組51例，在兩組接受基礎治療的前提下，對照組在一般治療基礎上加用水飛薊賓膠囊，2粒/次，3次/d，飯后服用。治療組同時加用加味理沖湯，1 劑/d，療程8周。比較兩組治療前后臨床癥狀、體征、血脂指標及影像改變情況。 結果 2組治療后肝功能指標ALT、AST、r-GT均較治療前顯著改善（P<0.05），且治療組改善效果優于對照組（P<0.05），治療組總有效率80.4%，高于對照組56.9%，對比差異具有統計學意義（P<0.05）。結論 加味理沖湯治療非酒精性脂肪肝能夠降低血脂，改善肝功能，減輕肝臟脂肪病變。

[Abstract] Objective To prove that the modified Li Chong Decoction in the treatment of non alcoholic fatty liver； clinical. Methods Group selection a randomized controlled trial were included in March 2014 to March 2016 in our hospital 102 cases of non-alcoholic fatty liver disease， the treatment group 51 cases， 51 cases in the control group， the treatment groups receiving basic premise under the control group received general treatment based on water silibinin capsule， a two， three times a day after meals. The treatment group added in processing punch flavored soup， one dose per day， for 8 weeks. Clinical symptoms， signs， serum lipids and imaging changes before and after treatment were compared. Results After treatment， liver function ALT， AST， r-GT than those before treatment significantly improved（P <0.05）， and the effect of improving the treatment group than the control group（P <0.05）. Treatment group， the total efficiency of 80.4%， higher than 56.9%， compared to a statistically significant difference（P <0.05）. Conclusion Modified Lichong Decoction in the treatment of non alcoholic fatty liver disease can reduce blood fat， improve liver function， reduce fatty liver disease.

非酒精性脂肪性肝病 （NAFLD）是一種以肝實質細胞脂肪變性、脂肪貯存為特征的臨床病理綜合征，排除過量飲酒史，與代謝綜合征關系密切[1]?，F代醫學治療非酒精性脂肪肝不良反應較多，中醫通過辨證論治的個體化治療，發揮中醫藥綜合調理作用，能達到良好預期效果，且并發癥少，因此具有廣闊的前景。非酒精性脂肪肝屬于中醫學“脅痛”、“肝著”等病癥范疇。我們通過查閱相關文獻總結認為，非酒精性脂肪肝的基本病機為正氣虧虛于內，造成血行瘀滯，兼有痰濕內蘊，故其病機為正虛邪實[2]。繼承前人經驗的同時結合臨床實踐，給予加味理沖湯治療，療效頗佳。通過隨機研究設計，整群選取2014年3月—2016年3月期間于該院住院治療的102例非酒精性脂肪性肝病患者作為研究對象，探討中西醫聯合用藥的臨床效果，現報道如下。……
