斜T形鎖定加壓接骨板治療橈骨遠端骨折的臨床分析

[摘要] 目的 分析斜T形鎖定加壓接骨板治療橈骨遠端骨折療效。方法 方便選取從2013年1月—2016年1月收治的AO分類B型與C型82例橈骨遠端骨折患者，均采取斜T形鎖定加壓接骨板治療，分析其治療效果。結果 該組82例患者，術后均獲得隨訪，隨訪時間平均為（8.43±1.09）個月；X線檢查顯示，骨折完全愈合，橈骨軸向無縮短，隨訪過程中，未出現骨折再移位與固定釘板松動情況。該組中，70例優，12例良，優良率為100%。結論 應用斜T形鎖定加壓接骨板治療橈骨遠端骨折，效果明顯，手術對腕關節血運破壞小，能恢復橈骨遠端解剖形態，有助于術后腕關節的功能恢復。

[Abstract] Objective To analysis of skew T-shaped locking compression plate in treatment of distal radius fractures. Methods Convenient selected from January 2013 to January 2016 from the AO classification of B type and C type 82 cases of distal radius fracture patients were taken with oblique T shape locking compression plate in treatment， to analyze the therapeutic efficacy. Results This group 82 cases of patients， surgery were followed up. The follow-up time averaged （8.43±1.09） months. X-ray examination showed complete fracture healing， radial axial shortening and follow-up process without fracture displacement and the fixed nail plate loosening. The group， 70 cases were excellent， 12 cases were good， the excellent and good rate was 100%. Conclusion The application of oblique T shape locking compression plate in treatment of distal radius fracture， significant effect， side injury， blood transport damage， can maintain the operation back to anatomy， restored in the affected limb.

臨床中，橈骨遠端骨折屬于多發骨折，在全身骨折中，其發生率占1/6，是一種較為常見的上肢骨折[1-4]。目前，在橈骨遠端骨折治療中，傳統手法復位石膏托外固定方法較多，對于橈骨遠端關節外簡單骨折，該方法效果較為理想，但對于不穩定橈骨遠端粉碎性骨折，特別是關節內骨折，傳統手法復位石膏托外固定方法，難以實現關節面穩定固定與良好對位，容易導致橈尺關節以及橈腕骨性關節炎。最近幾年以來，隨著內固定技術水平的不斷提高，人們對于腕關節功能康復要求越來越高，內固定手術在橈骨遠端粉碎性骨折治療中得以應用。該文主要分析斜T形鎖定加壓接骨板（locking-compression plate，LCP）治療從2013年1月—2016年1月收治的AO分類B型與C型82例橈骨遠端骨折臨床療效，效果滿意，現報道如下。……
