尿道損傷患者修復重建術后性功能狀況調查分析

【摘要】目的：探討尿道損傷患者修復重建術后性功能狀況及危險因素，以期注重防范，提高臨床效果。方法：隨機選取2011年1月至2013年4月76例尿道損傷患者為研究對象，均予修復重建術，觀察術后在性功能上變化情況，并對受影響的患者進行危險因素分析，采用非條件Logistic回歸法進行多因素分析。結果：術前IIEF-5評分平均為（23.13±2.14）分，術后IIEF評分平均為（11.14±1.32）分，術后1年有21例患者出現性功能障礙，發生率為27.63%；危險因素中，單危險因素有文化程度低（高中以下）、年齡?。?30歲）、婚齡短（<10年）、配偶態度、性觀念和性心理消極、伴粉碎性骨盆骨折、醫師年資低（<10年）、悲觀心理（P<0.05）；而和尿道損傷類型無關（P>0.05）。其中年齡、文化程度、婚齡、配偶態度、性觀念和性心理情況、骨盆骨折類型、醫師年資、心理狀態是術后性功能障礙獨立危險因素（P均<0.05）。結論：尿道損傷后患者修復重建術對患者的影響因素較多，在臨床上要加強防范。

【Abstract】Objectives： To investigate the sexual function status and risk factors of patients after urethral reconstructive surgery， in order to improve the prevention and clinical results. Methods： 76 urethral injury patients from January 2011 to April 2013 were randomly selected and correspondingly given reconstructive surgery. Changes in sexual function were observed. Risk factors were analyzed using Logistic regression method for multivariate analysis. Results： The preoperative IIEF-5 score was （23.13 ± 2.14） points and postoperative IIEF score was （11.14 ± 1.32） points. 21 patients had sexual dysfunction a year after treatment， the incidence rate of 27.63%. The risk factors included low level of education （high school or less）， younger age （<30 years）， short marriageable age （<10 years）， spouses attitudes， sexual attitudes and passive sex psychology， having comminuted fracture， low doctors seniority （<10 years） and pessimistic psychology （P<0.05）； regardless of urethral injury type （P>0.05）， where age， education level， age of marriage， spouse attitudes， sexual attitudes and psychological conditions， pelvic fracture type， physicians seniority， mental state postoperative sexual dysfunction are independent risk factors （P<0.05）. Conclusion： Many factors may influence patients sexual function after urethral repair reconstructive surgery， which should be paid more attention to in clinic.

隨著社會經濟的發展，我國各種交通事故傷在臨床上發病率越來越高，對一些骨盆骨折患者往往容易造成尿路挫傷或斷裂，加上各種微創損傷的醫源性損傷引起尿道損傷的發生率也不斷升高。目前尿道損傷修復重建術后患者往往會出現性功能障礙已經成為一個關鍵問題，臨床醫師也從以往的僅關注手術逐漸也關注對術后性功能狀況的影響性[1]。本次研究就通過觀察尿道損傷修復重建術后對患者性功能狀況的影響性，以期提高臨床診治水平。

1資料與方法

1.1臨床資料

隨機選取2011年1月至2013年4月76例尿道損傷患者為研究對象，年齡最小19歲，最大60歲，平均（38.3±2.8）歲；損傷原因：交通事故傷35例，高處墜落傷20例，其他傷21例?！?br>
