【摘要】目的:觀察氬氣高頻電刀聯合干擾素治療尖銳濕疣的療效以及對淋巴細胞趨化(LTN)因子和白細胞介素-17(IL-17)的影響。方法:將本院收治的尖銳濕疣患者96例參照數字法隨機分為治療組和對照組,兩組各48例;對照組給予氬氣高頻電刀治療,同時口服蒲地藍消炎膠囊,3粒/次,4次/d;治療組在對照組基礎上采用重組人干擾素α-2b肌內注射治療,1×106IU/d,3次/d;兩組均連續治療4周。分析兩組治療后臨床療效;比較兩組不良反應和復發情況;檢測兩組血清LTN和IL-17。結果:治療組患者臨床總有效率為89.58%,對照組為66.67%,治療組明顯高于對照組(P<0.05);兩組治療期間不良反應發生比較均無顯著差異(P>0.05);對照組在治愈基礎上有7例復發,治療組復發3例,治療組明顯低于對照組(P<0.05);治療組治療后2周和4周,患者血清LTN水平明顯高于同期對照組,IL-17明顯低于同期對照組,比較差異有統計學意義(P<0.01)。結論:氬氣高頻電刀聯合干擾素治療尖銳濕疣的療效顯著,不良反應少,復發率低,提高患者血清LTN水平和降低血清IL-17水平可能與上述療效相關。

【Abstract】Objectives: To observe the curative effect of argon high-frequency electrocautery combined with interferon in treating verruca acuminate and influences on lymphotactin and interleukin-17. Methods: According to the random number table method, 96 patients with verruca acuminate in our hospital were randomly divided into control group (48 cases) and treatment group (48 cases). Patients in control group were orally treated by argon high-frequency electrocautery and Pudilan Xiaoyan capsule (3 capsules/time and q6h). On such basis, treatment group was treated by recombination interferon α2b by intramuscular injection (1×106IU/d, tid). The therapy courses were 4 weeks in both groups. Clinical effects were analyzed between both groups after treatment. Adverse reactions and recurrences were compared in two groups. Serum LTN and IL-17 were detected for both groups. Results: The total effective rate for treatment group was 89.58%, which was superior to control group of 66.67%(P<0.05). Adverse reactions of two groups had no significant differences (P>0.05). There were 7 cases of recurrences in control group, which was less to treatment group of 3 cases (P<0.05). Serum LTN level of treatment group was obviously higher those of control group, while IL-17 was evidently lower, with statistic differences after 2 and 4 weeks treatment (P<0.01). Conclusion: Argon high-frequency electrocautery combined with interferon in treating verruca acuminate has a striking effect with rare adverse reactions and low recurrence rates, and its role may be related with increasing serum LTN and decreasing IL-17.endprint

尖銳濕疣屬性傳播疾病,由人乳頭瘤病毒(HPV)感染引起的生殖器、會陰以及肛門及周圍部位的表皮增殖性病變。由于其病變部位的特殊性,對患者的生活質量和心身健康構成嚴重威脅[1]。調查顯示,尖銳濕疣的發病率呈逐年上升趨勢,且具有生長速度較快、復發率高等特點,給臨床治療帶來極大挑戰[2]。目前,臨床治療尖銳濕疣的療法較多,如口服藥物、手術切除以及化學治療等,但療效欠佳,治愈后復發率高[3]。
研究[4,5]發現,在尖銳濕疣治療中,抗HPV 感染是其治本療法。為了達到標本兼治的目的,本研究在干擾素抗HPV治療基礎上,給予氬氣高頻電刀根除治療,取得良好治療效果;……
