三陰交——合谷穴艾灸對產程及產后出血量的影響

【摘要】目的：觀察艾灸三陰交-合谷穴對產程和產后出血量的影響。方法： 200例初產婦，采用單盲、隨機方法分為三陰交-合谷組、三陰交組、合谷組、非穴位組和空白組5組，每組各40例。三陰交-合谷組在產婦出現規律性宮縮痛、宮口開大3cm時行三陰交穴聯合合谷穴施灸30min；三陰交組在三陰交穴施灸30min；合谷組在合谷穴施灸30min；非穴組在腕橫紋橈側端橈動脈搏動處至肘橫紋肱二頭肌腱橈側端連線的中點處施灸30min；空白組不做艾灸干預。分別對產婦第一產程、第二產程、第三產程及總產程時間及娩出胎盤后出血量、產后2h出血量及產后24h出血量進行比較。結果：三陰交-合谷組與空白組第一產程、第二產程、第三產程比較差異有統計學意義；三陰交組與空白組第一產程比較差異有統計學意義；合谷組與空白組第二產程比較差異有統計學意義。三陰交-合谷組與空白組娩出胎盤后出血量、產后2h出血量及產后24h出血量比較差異有統計學意義；三陰交組與空白組產后2h出血量比較差異有統計學意義；合谷組與空白組娩出胎盤后出血量及產后24h出血量比較差異有統計學意義，其余比較差異均無統計學意義。結論：艾灸三陰交穴、合谷穴艾灸對產婦產程的影響效果有效，但聯合穴位艾灸更能縮短產程、促進產程進展、減少產后出血量，是安全的催產方法。

【Abstract】Objectives： To observe the effect of moxibustion at sanyinjiao-hegu on the stage of labor and postpartum hemorrhage. Methods： 200 primiparas， using single blind method， were randomly divided into the sanyinjiao group， the hegu group， sanyinjiao-hegu group， non-acupuncture group and the blank group， each of 40 cases. Patients in the sanyinjiao-hegu group received moxibustion for 30min at sanyinjiao-hegu， when regular uterine contraction pain occurred and the ostium of uterus was over 3cm. The sanyinjiao group and hegu group received moxibustion at sanyinjiao and hegu respectively for 30min. The non-acupuncture group was exerted moxibustion therapy at the midpoint of the radial side of wrist horizontal stripes of radial side radial pulse place to elbow horizontal stripes biceps tendon for 30min， while the blank group had no moxibustion intervention. The time of each labor stage and whole delivery time as well as the postpartum hemorrhage were compared between different groups. Results： There was significant difference between the sanyinjiao-hegu group and blank group in the time of first， second and third labor stage， so as the postpartum hemorrhage； there was significant difference between the sanyinjiao group and blank group in the time of first labor stage， so as the difference in postpartum hemorrhage 2 hours after delivery； there was significant difference between the hegu group and blank group in the time of second labor stage， so as the difference in postpartum hemorrhage and that 24 hours after delivery. Conclusion： Moxibustion at sanyinjiao and hegu acupoint can shorten the duration of labor stage， especially joint moxibustion which will promote the production process and reduce postpartum hemorrhage.endprint
