【摘要】目的:肝素結合表皮生長因子(HB-EGF)在女性生殖系統中有非常重要的作用,它能促進子宮內膜從增殖期向分泌期轉化,并且介導受精卵著床,促進胎盤的形成和生長,同時也有抑制凋亡的作用。本研究意在探討HB-EGF在稽留流產的病因中所起到的作用。方法:選取意外妊娠的患者12例作為對照組,稽留流產的患者13例為實驗組,采用人流術或刮宮術取得子宮蛻膜組織和胎盤組織,利用免疫組織化學染色的方法檢測實驗組和對照組的蛻膜組織與胎盤組織里的HB-EGF的水平。結果:稽留流產患者的蛻膜組織與胎盤組織內的 HB-EGF表達均有上調[分別是(143.31±12.50)和(115.72±12.13)],并且明顯高于對照組的蛻膜組織與胎盤組織內的 HB-EGF表達[分別是(102.40±13.21)和(82.30±9.97)]。結論:研究數據表明HB-EGF的高表達在稽留流產中可能是導致死胎滯留腹中的原因。

【Abstract】Objectives: Heparin-binding epidermal growth factor(HB-EGF)has important biologic functions in the female reproductive tract. It helps to promote the uterine from proliferation stage to secretory stage, and has functions in implantation, placenta development, and also inhibition of apoptosis. This research aims to explore the possible role of HB –EGF in missed abortion. Methods: Decidual and placental tissue samples were obtained from 12 women with unwanted pregnancy as the control group and from 15 women with missed abortions as the experiment group. Immunohistochemistry was used to compare HB- EGF expression in uterine decidual tissue and in placental villous stroma of the two groups. Results: HB-EGF expression in both uterine decidual tissue and placental villous stroma was up-regulated in the missed abortion group (143.31±12.50 and 115.72±12.13 respectively), compared with the control group (102.40±13.21 and 82.30±9.97102.4, respectively). Conclusion: The up-regulated expression of HB-EGF may inhibit the discharge of dead fetus in missed abortions.

稽留流產又名過期流產,指宮內胚胎或胎兒死亡后未及時排出者,它屬于一種特殊的自然流產[1]。近年來,稽留流產的發病率逐年增加[2]。造成稽留流產的原因較多,如染色體的數量或結構等異常、子宮畸形、嚴重營養缺乏、環境不良因素及精神因素等[3]。前人在稽留流產的診斷及治療方面做了很多相關性研究,但對于稽留流產的病因研究卻較少,因此本文旨在對稽留流產的病因做一探討。
作為表皮結合因子(EGF)家族的一員,肝素結合表皮生長因子(HB-EGF)基因是位于人類第5號染色體長臂上,分子質量為20~22ku, 由208個氨基酸構成[4]。……
