【摘要】 目的 研究排石顆粒治療感染性輸尿管上段尿路結石的臨床療效。方法 60例感染性輸尿管上段尿路結石患者, 隨機分為實驗組和對照組, 各30例。兩組患者均給予常規抗感染治療, 對照組僅給予抗生素治療, 實驗組給予排石顆粒聯用抗生素治療。觀察兩組結石位置、腎絞痛再次發生率等。結果 實驗組結石下移(2.8±0.5)cm優于對照組的(1.2±0.6)cm, 差異有統計學意義(P<0.05)。實驗組腎絞痛再次發生率為6.7%低于對照組的30.0%, 差異有統計學意義(P<0.05)。結論 排石顆粒聯合抗生素使用于感染性輸尿管上段尿路結石治療, 有助于減少腎絞痛發作, 療效肯定, 便于推廣應用。

【Abstract】 Objective To study the clinical efficacy of Paishi granules for infectious urinary stones in upper ureter. Methods A total of 60 patients with infectious urinary stones in upper ureter were randomly divided into experimental group and control group, with 30 cases in each group. Both groups received conventional anti-infective treatment, and the control group was treated with antibiotics. The experimental group was treated with Paishi granules and antibiotics. Observation were made on stone position and renal colic recurrence rate in two groups. Results The experimental group had better stone displacement as (2.8±0.5) cm than (1.2±0.6) cm in the control group, and the difference was statistically significant (P<0.05). The experimental group had lower renal colic recurrence rate as 6.7% than 30.0% in the control group, and the difference was statistically significant (P<0.05). Conclusion Combination of Paishi granules and antibiotics provides affirmative efficacy in treating infectious urinary stones in upper ureter, and it helps reduce occurrence of renal colic attack. So it is easy to be popularized and applied.

感染性結石是尿路結石中常見的一種類型, 約占泌尿系結石的 5%~15%[1, 2], 感染性結石在腎臟常形成鹿角形結石, 其生長速度快, 如不積極治療, 演變為慢性腎盂腎炎, 甚至可導致尿源性敗血癥[3, 4]。由于其復發率高, 外科治療風險大, 且術后抗生素治療周期長, 導致患者的依從性較差。臨床上一直希望能夠找到一種對感染性結石療效理想且能長期使用, 同時又能避免嚴重不良反應的療法[5, 6]。感染性結石屬于中醫“石淋”、“熱淋”的范疇[7]。尿中沉濁物與濕熱互結而成砂石, 所以在治療上應以清熱利濕、排石通淋為主。目前中醫藥療法以其療效好、價格低廉等特點被眾多患者所接受[8, 9], 本研究擬通過觀察與評估排石顆粒對治療上尿路感染性結石術后的臨床療效, 報告如下。
1 資料與方法
