耳廓軟骨聯合硅膠假體置入修復隆鼻術后并發癥

[摘要] 目的 探討修復隆鼻術后并發癥的方法。 方法 對30例隆鼻術后出現并發癥的患者采用鼻部假體取出，并聯合應用耳廓軟骨鼻尖成形及硅膠假體置入重新隆鼻的術式進行修復。 結果 本組30例患者均采用上述術式，經過1～2年的隨訪，二次修復后，原并發癥消除，鼻部形態基本滿意。 結論 聯合應用耳廓軟骨鼻尖成形及硅膠假體置入隆鼻是一種修復隆鼻術后并發癥的有效方法。

[Abstract] Objective To explore the method of repairing the complications of rhinoplasty. Methods Thirty patients with complications after rhinoplasty were repaired by using auricular cartilage combination with silicone prosthesis. Results Satisfactory results were obtained after 1 to 2 years followed-up，and the original complications were eliminated. Conclusion The method is effective for the treatment of the complication of rhinoplasty.

隆鼻術目前在頭面部美容手術中的比例已占第2位[1]，其中固體硅膠假體隆鼻術因造價低、操作簡單而被廣泛應用， 但其術后并發癥也不少見。據報道其術后并發癥及效果不滿意者發生率為4.0%～20.8%[2]。2008～2010年，對30例隆鼻術后出現各種并發癥的患者，采取鼻部假體取出，同時應用耳廓軟骨鼻尖成形及硅膠假體置入重新隆鼻的術式進行修復，術后獲得較滿意效果，現報道如下。

1 資料與方法

1.1 一般資料

本組30例患者，男5例，女25例，年齡19～46歲，其中假體偏斜或斷裂6例，假體浮動7例，鼻尖皮膚張力過大或假體外露6例，假體輪廓不滿意5例，感染4例，假體排異2例。采用鼻部假體取出，同時應用聯合耳廓軟骨鼻尖成形及硅膠假體置入重新隆鼻術，術后鼻部適當延長拆線時間，7～8 d拆線，耳部14 d拆線，隨訪1～2年，效果滿意，鼻部形態自然美觀。

1.2 手術方法

1.2.1 鼻部假體取出 常規眶下孔、鼻部局部麻醉后，一般采取右側鼻孔緣切口入路，沿右側大翼軟骨表面潛行分離至原植入物，銳鈍結合分離取出原植入物，可為硅膠、膨體、羥基磷灰石、液體硅膠等。……
