自體脂肪顆粒在矯正顳部凹陷中的應用

[摘要] 目的 觀察總結自體脂肪顆粒注射矯正顳部凹陷美容術29例的臨床效果。 方法 在腫脹麻醉下采用注射器法從供區抽取自體脂肪液經清洗、純化后將自體脂肪顆粒注射移植到顳部凹陷部位。 結果 本組29例自體脂肪顆粒注射移植后，23例一次注射移植矯正成功，占79.3%；6例脂肪注射移植后效果欠佳，占20.7%，3個月后經第二次脂肪顆粒移植后效果滿意。經3～12個月的隨訪觀察，顳部凹陷填充后，塑形效果良好，患者顳部豐滿，臉型較術前柔和、協調。結論 自體脂肪顆粒注射矯正顳部凹陷可取得較好的臨床效果，是一種安全、簡便、有效的手術方法。

Autologous fat granule in the correction of temporal depression

[Abstract] Objective To observe the clinical effect of the autologous fat granule injection for the correction of temporal depression. Methods In tumescent anesthesia，inject the aotulogous fat granule to the sunken temloral area，which extracted from supply area by syringe，after cleaning and purification. Results This group of 29 patients with autologous fat granule injection，23 patients were success in one-time， accounted for 79.3%；In 6 cases with poor effect，accounted 20.7%，which were success after the second fat granule transplantation after 3 months.From 3 to 12 months follow-up，the temporal depression after filling， shaping effect is good，in patients with temporal plump，the shape of the faces were soft，coordination than preoperative. Conclusion The operation method of the autologous fat granule injection for the correction of temporal depression can achieve the better clinical results， which is safe，simple and effective.

顳部凹陷多為先天發育不良所致。臨床上常見菱形臉的人前額較長，顴部相對過高，可能與兒時缺鈣引起顱骨發育不良有關，也可能屬于遺傳因素。在臨床上要求做顳部充填的人不少，筆者所在醫院自2008年4月～2010年3月，通過自體脂肪顆粒注射顳部充填后，可使患者頭面部的曲線變得協調、勻稱，臉型更美。

1 資料與方法

1.1 一般資料

本組患者29例，其中女21例，男8例，年齡24～55歲，采用注射器抽取自身脂肪后，經過沖洗過濾，將脂肪顆粒注射至顳部凹陷區，充填量為5～15 mL（單側）。

1.2 手術方法

1.2.1 取脂部位和方法 用美蘭標記出顳部凹陷的范圍及脂肪供區的抽吸范圍。供脂區一般選擇局部脂肪堆積較多的部位，如腹部、腰部、臀部，以股前外側為最佳[1]。選用腫脹麻醉：按2%利多卡因30 mL加0.1%鹽酸腎上腺素1 mL加5%碳酸氫鈉20 mL加0.9%氯化鈉注射液1 000 mL的比例配制。……
