昭通市第一輪全球基金結核病控制項目實施效果分析

[摘要] 目的 分析第一輪全球基金昭通市結核病控制項目實施效果。 方法 根據2003～2007年項目縣、區上報的中國全球基金季報表、中國疾病預防控制系統結核病管理信息系統數據，進行統計分析。 結果 2004年項目覆蓋10個貧困縣（區），覆蓋率達100%；新涂陽肺結核患者發現率達127.22%；涂陽患者治療2、3個月末痰菌陰轉率為88.46%和95.75%；涂陽肺結核病治愈率為91.76%。 結論 項目實施效果較好，各項目指標均達到項目設計要求。

Analysis effect of the global fund tuberculosis control project in Zhaotong city in the first round

[Abstract] Objective To analyze the effect of the global fund tuberculosis control project in Zhaotong city in the first round. Methods The article is analyzed on the basis of the China fund quarterly and the tuberculosis management information system data of the China disease prevention and control system that are reported by each county and district from 2003 to 2007. Results In 2004，the project covers 10 impoverished counties（regions）and the cover degree was up to 100%.The case-detection rate of the new smear-positive pulmonary tuberculosis patients reached 127.22%.The sputum negative conversion rate of smear positive patients was 88.46% and 95.75% at the end of February and March and the cure rate of the smear-positive pulmonary tuberculosis was 91.76%. Conclusion The effect of the project is good. Each project indicator has reached the design requirements.

昭通市地處云南東北部，位于北緯26°34′～28°40′與東經102°52′～105°19′之間，總面積22 430 km2，分別與四川省和貴州省相接壤，多數為山區。全市轄11縣（區），35個鎮，134個鄉（其中19個民族鄉），4個城區辦事處，1 221個村（居）委會和辦事處，25 805個村民小組。全市總人口 524.35萬，其中92%以上為農業人口，共有23種民族，少數民族人口占總人口的10.25%，人口較多的有彝族、苗族、回族；貧困縣10個，占了全市的90%以上。農民人均純收入2 768元。

1 資料與方法

1.1 一般資料

資料來源于2003～2007年昭通市全球基金項目單位的10縣、區上報的中國全球基金季報表。

1.2 方法

各縣、區上報的中國全球基金季報表數據導入Excel表進行統計分析。肺結核病診斷、抗酸桿菌檢驗、治療判斷標準均按照《中國結核病防治規劃實施工作指南2002》執行[1]。

2 結果

2.1 結核病疫情

2.1.1 歷次流行病學調查的結核病患病率 昭通市被抽為國家結核病流行病學調查點。在1990年流行病學調查中，昭通市活動性肺結核患病率為398.1/10萬，涂陽患病率為66.3/10萬；在2000年的流行病學調查結果為活動性肺結核患病率為1 659.2/10萬，涂陽患病率為446.71/10萬，調查反映筆者所在城市結核病疫情已十分嚴重，且呈上升趨勢。……
