VSD結合Oream護理在慢性骨髓炎引流中的治療研究

[摘要] 目的 探討運用負壓封閉引流（vacuum sealing drainage，VSD）結合Oream護理在慢性骨髓炎引流中的療效研究。方法 收集2008～2013年100例慢性骨髓炎患者，按照隨機數字表均分為兩組。研究組在Oream護理下予VSD引流，對照組常規手術后予一次性負壓吸引器引流，比較兩組引流量、置管時間、換藥次數、住院時間、換藥VAS評分和生活質量各方面的評分等方面。結果 研究組術后引流量、置管時間、換藥次數、住院時間、換藥VAS評分和生活質量各方面的評分等方面均優于對照組，差異有統計學意義（P<0.05）。結論 運用VSD結合Oream護理在慢性骨髓炎引流中有較好的效果，提高醫療護理質量，值得臨床推廣。

Clinical research of VSD combined Oream nursing on the drainage of chronic osteomyelitis

[Abstract] Objective To investigate the curative effect of the VSD combined Oream nursing on the chronic osteomyelitis drainage. Methods Collected 100 cases of patients with chronic osteomyelitis from 2008 to 2013， sorted by hospital successively， who were divided into the research group and the control group randomly. The research group was given the VSD drainage and the Oream nursing， while the control group given routine to one-time negative pressure aspirator drainage after surgery， compared the two groups of traffic and catheter time， switching frequency， length of hospital stay， treatment in all aspects of the VAS score and quality of life score， etc. Results The flow rate， the time of insertion， switching times， length of hospital stay， treatment in all aspects of the VAS score and quality of life scores of the research group were better than that in control group， the difference was statistically significant （P<0.05）. Conclusion The VSD combined with Oream nursing has good effect， improving the quality of nursing and patients satisfaction.

慢性骨髓炎局部皮膚軟組織反復潰爛合并缺損，滲出大量膿性組織，創面長期不愈合，骨質缺損破壞嚴重，骨外露或竇道形成，即使長時間抗感染治療效果較差，甚至加重骨壞死，預后較差[1]。慢性骨髓炎的治療常規放置引流，但具體引流材料和方法各異，療效參差不齊[2]。負壓封閉引流（vacuum sealing drainage，VSD）能夠去除分泌物、腔隙和壞死組織，有利于愈合[3]。奧瑞姆自理模式強調最終的護理目標是增強并恢復患者自理能力。Oream護理定義：按照治療目標最大程度地維持并促進患者自理能力[4]，主要分為三大系統：全補償系統（無任何自理能力）、部分補償系統（能滿足部分自理，但需要醫護滿足另一部分）和輔助教育系統（在咨詢、指導教育下能夠完成大部分自理需求）。本研究觀察運用VSD結合Oream護理在慢性骨髓炎引流中的療效，現報道如下。
