[摘要] 目的 探討血清C反應蛋白、脂蛋白a及尿酸水平與臨床高血壓病合并頸動脈粥樣硬化的相關性。方法 選用來該院全科門診高血壓患者123例采用B型超聲測定頸動脈中層厚度(IMT),根據粥樣硬化程度分為正常組41例,IMT增厚組為82例。測定患者體內血清C反應蛋白、脂蛋白a及尿酸水平濃度變化,分析其與高血壓病合并動脈粥樣硬化相關性。結果IMT增厚組與IMT正常組體內三者含量差異有統計學意義(P<0.05)。對IMT增厚組患者體內三者含量與頸動脈中層厚度進行相關性分析,三者與頸動脈中層厚度(IMT)具有相關性(P<0.05)。且相互之間也存在相關性(P<0.05)。 結論 高血壓病合并頸動脈粥樣硬化患者體內血清C反應蛋白、脂蛋白及尿酸水平與頸動脈中層厚度(IMT)關系密切,臨床可以作為診斷依據。

The Correlation of Levels of Serum C-reactive protein, Lipoprotein (a) and Uric Acid with Hypertension Complicated by Carotid Atherosclerosis

[Abstract] Objective To investigate the correlation of levels of serum C-reactive protein, lipoprotein(a) and uric acid with hypertension complicated by carotid atherosclerosis. Methods 123 cases with hypertension visited the General Practice Clinic of our hospital were selected. B-mode ultrasonography was used to measure the carotid intima-media thickness(IMT) in the patients. In accordance with the degree of atherosclerosis, the patients were divided into the IMT normal group(41 cases), and IMT thickening group(82 cases). And the levels of serum C-reactive protein, lipoprotein(a) and uric acid were measured in the patients, and the correlation of them with hypertension complicated by carotid atherosclerosis was analyzed. Results There were significant differences in the levels of serum C-reactive protein, lipoprotein(a) and uric acid between the IMT thickening group and the IMT normal group (P<0.05). The analysis was conducted on the correlation of the levels of serum C-reactive protein, lipoprotein(a) and uric acid with the carotid intima-media thickness(IMT) in the IMT thickening group, and the result showed that the levels of serum C-reactive protein, lipoprotein(a) and uric acid had correlation with the carotid intima-media thickness(IMT) (P<0.05), and there was also a correlation between serum C-reactive protein and lipoprotein(a) or serum C-reactive protein and uric acid, or lipoprotein(a) and uric acid(P<0.05). Conclusion The levels of serum C-reactive protein, lipoprotein and uric acid in patients with hypertension complicated by carotid atherosclerosis are closely related to the carotid intima-media thickness(IMT), which can be used as the basis for the clinical diagnosis.

動脈粥樣硬化[1]是動脈硬化血管病常見的一種,癥狀主要決定于血管病變及受累器官的缺血程度,臨床上高血壓患者合并動脈粥樣硬化發病率明顯增高。給患者身體健康帶來了極大危害,研究表明[2]臨床血清C反應蛋白、脂蛋白a及尿酸水平與高血壓病合并頸動脈粥樣硬化關系密切與心血管疾病有關。該次研究通過運用B型超聲測定頸動脈中層厚度(IMT)和測定血清C反應蛋白、脂蛋白和尿酸濃度,來探究高血壓病合并頸動脈粥樣硬化與三者含量的相關性,現分析2011年1月—2013年4月間來該院全科門診高血壓患者123例的臨床資料,報道如下。
1 資料與方法
1.1 一般資料
隨機選取來該院全科門診高血壓患者123例,其中男67例,年齡在28~75歲,平均年齡在(39.7±10.2)歲,女56例,年齡在27~79歲,平均年齡在(41.2±9.51)歲。所有高血壓患者經B型超聲測定頸動脈中層厚度<0.9 mm為IMT正常組,≥0.9 mm為IMT增厚組,IMT正常組41例,男22例,女19例,平均年齡在(42.1±9.9)歲,IMT增厚組82例,男45例,女47例,平均年齡在(40.8±11.0)歲。
1.2 納入標準
①所選高血壓患者均隨機選取,經臨床診斷均符合中國高血壓防治指南(2005年)高血壓診斷標準?!?br>
