臨床藥學服務平臺的構建與應用

摘 ? 要：基于醫院信息平臺構建臨床藥學服務平臺，與醫生工作站系統、檢驗信息系統、醫學影像信息系統及電子病歷系統等互聯互通，方便臨床藥師實時查詢患者的電子病歷、用藥醫囑、檢驗和檢查結果等診療信息，支持藥師主動參與臨床診療過程，干預和指導臨床用藥，實現與醫生的互動，保障臨床用藥的安全和合理。

Abstract：Based on the hospital information platform， the clinical pharmacy service platform is constructed， which is interconnected with the doctor workstation system， the inspection information system， the medical imaging information system and the electronic medical record system， so that the clinical pharmacist can query the patient's electronic medical record， medication instructions， inspection and examination results in real time. Information， support pharmacists to actively participate in the clinical diagnosis and treatment process， intervention and guidance of clinical medication， to achieve interaction with doctors， to ensure the safety and rationality of clinical medication.

目前我國醫院藥學正經歷著從“藥品的保障供給為中心”轉向“以患者為中心的藥學服務”的延伸與發展階段[1]，而在醫院信息系統現有的應用體系中，沒有為臨床藥師這個角色提供專業、高效和全面支持臨床藥學服務的信息系統工具。現有的合理用藥系統、臨床藥學系統，因設計的局限性，未能與以電子病歷為核心的臨床信息系統充分融合，從而未能有效支持臨床藥師在干預臨床用藥等方面發揮積極作用，致使臨床藥師在醫療機構中角色缺位。為此，研發臨床藥師系統，構建臨床藥學服務平臺[2]，支持臨床藥師參與臨床藥物治療，探討藥物合理化應用、實施個體化給藥、檢測藥品不良反應和開展藥物經濟學[3]相關研究，構建醫院藥品使用安全體系等，具有重要意義和推廣價值。

1功能設計

1.1獲取診療信息 ?通過醫院信息平臺，實現臨床藥學服務平臺與以電子病歷為核心的臨床信息系統的互聯互通，實時獲取患者的用藥醫囑、檢驗報告、病理報告、檢查報告和電子病歷等信息，支持臨床藥師干預和指導臨床用藥。……
