少數民族地區家庭醫生簽約服務模式的探索與實踐

[摘要] 家庭醫生簽約服務工作是推進醫藥衛生體制綜合改革，打造健康中國，促進分級診療工作實施推廣的一項非常重要的舉措。該文介紹了以鄂爾多斯為列的少數民族地區推進家庭醫生簽約服務工作的主要做法和特點，提出存在的問題和深入實施開展此項工作的政策建議，以期為實現基層首診、急慢分診、上下聯動、雙向轉診的分級診療格局提供實踐和理論借鑒。

[Abstract] Family doctor contracting service work is a very important measure to promote the comprehensive reform of the medical and health system， build a healthy China， and promote the implementation of hierarchical diagnosis and treatment. This article introduces the main practices and characteristics of promoting family doctor contracting services in ethnic minority areas such as Ordos， presents existing problems， and implements policy recommendations for in-depth implementation of this work， with a view to achieving first-level consultation at the grassroots level， urgent and slow triage， to provide practical and theoretical reference for the hierarchical diagnosis and treatment pattern of up-down linkage and two-way referral.

家庭醫生簽約服務是推進基層衛生綜合改革、實現分級診療、合理優化衛生資源、構建和諧醫患關系、加強疾病預防控制的重要途徑[1]。習近平總書記在2016年全國衛生健康大會上明確指“要把人民健康放在優先發展的戰略地位”，緊接著國務院出臺了《關于推進家庭醫生簽約服務的指導意見》，強調家庭醫生簽約服務制度是新形勢下深化醫藥衛生體制改革和維護人民群眾健康的重要任務，也是轉變基層醫療衛生服務模式和強化基層醫療衛生服務網絡功能的重要途徑。家庭醫生簽約服務以全面健康管理為目標，通過契約服務的形式，由全科醫生、公共衛生、健康管理師等組成家庭醫生服務團隊，以街道社區為范圍，居民家庭為單位，為簽約的家庭和居民個人提供連續、安全、適宜的基本醫療、基本公共衛生和約定的個性化健康管理服務[2]。……
