[摘要] 目的 探討中性粒細胞與淋巴細胞比值與2型糖尿病腎病進展的相關性。方法 2014年9月—2016年12月間采用回顧性隊列研究納入2型糖尿病患者492例,據24 h尿蛋白總量將492例2型糖尿病患者分為糖尿病無腎病組(DM組)424例,糖尿病腎病組(DN組)68例。收集其臨床資料,追蹤其3年后DN進展情況。將基線中性粒細胞與淋巴細胞比值(NLR)水平按四分位法分為四組,即最低四分位數組(I組)≤1.37、第二四分位數組(II組)1.38~1.83、第三四分位數組(III組)1.84~2.21、最高4分位數組(IV組)≥2.22。按DN是否進展,將研究對象分為進展組(A組)和未進展組(B組),比較兩組基線資料,采用二元Logistic回歸,評估糖尿病病程、SBP、HbA1c、BMI及基線NLR水平與DN進展之間的相關性。結果3年后,64例(13.00%)患者糖尿病腎病進展,其中新發糖尿病腎病42例(8.53%)。單因素分析中,A組與B組相比較,年齡、病程、BMI、收縮壓、NLR、FPG、HbA1c、TC、TG差異有統計學意義(P<0.05)。結論NLR水平>1.83是T2DM患者DN進展的獨立危險因素。

[Abstract] Objective To investigate the correlation between the ratio of neutrophils and lymphocytes and the progression of type 2 diabetic nephropathy. Methods From September 2014 to December 2016 a retrospective cohort study was used to enroll 492 patients with type 2 diabetes. According to the total amount of 24 hours urine protein, 492 patients with type 2 diabetes were divided into diabetic nephropathy group (DM group) 424 and diabetic nephropathy group (DN group) 68 patients. Collect clinical data and track the progress of DN after 3 years. The baseline neutrophil-lymphocyte ratio (NLR) levels were divided into four groups according to the quartile, that is, the lowest quartile array (group I) ≤ 1.37, and the second quartile array (group II) 1.38 to 1.83; the third fourth quartile array (Group III) 1.84 to 2.21, the highest quartile array (Group IV) ≥ 2.22. According to whether DN progressed, the study subjects were divided into progress group (group A) and non-progressive group (group B). Baseline data of the two groups were compared, and binary logistic regression was used to evaluate the duration of diabetes, SBP, HbA1c, BMI, and baseline NLR levels, correlation with DN progress. Results After 3 years, diabetic nephropathy progressed in 64 patients (13.00%), including 42 newly diagnosed diabetic nephropathy (8.53%). In univariate analysis, there were significant differences in age, disease duration, BMI, systolic blood pressure, NLR, FPG, HbA1c, TC, and TG in group A compared with group B(P<0.05). Conclusion NLR level> 1.83 is an independent risk factor for DN progression in patients with T2DM.

糖尿病(diabetes mellitus,DM)是一種以血管和神經病變為主要并發癥的全身性疾病,慢性炎癥是2型糖尿病的特征,有研究表明[1],糖尿病患者的慢性炎癥促進了微血管病變的發生發展。糖尿病腎病(diabetic nephropathy,DN)是糖尿病患者中常見的微血管并發癥。終末期腎病,是導致糖尿病人群死亡的主要原因[2],24 h尿蛋白是診斷DN的重要指標之一。中性粒細胞與淋巴細胞比值(neutrophil to lymphocyte ratio,NLR)作為新興的炎癥標志物,近年來在心血管疾病及腫瘤預后等很多領域受到關注。研究表明[3],在新診斷的2型糖尿病患者中,NLR比值的增加與24 h尿蛋白和尿白蛋白排泄獨立相關。……
