牙體牙髓聯合正畸治療恒上前牙冠根折的療效分析及有效率評價

【摘要】目的：分析研究牙體牙髓聯合正畸治療恒上前牙冠根折的療效分析及有效率。方法：研究時段選取范圍2017年01月～2021年01月，入選此次研究100例患者均為我院接收治療的恒上前牙冠根折，結合此間恒上前牙冠根折患者應用治療方法的差異，將其分別納入對照組和觀察組，各50例，依次實施常規治療及牙體牙髓聯合正畸治療，比較在不同干預措施應用下的治療效果、并發癥發生率、生活質量、各項功能、VAS評分。結果：分析治療效果，經治療后，觀察組恒上前牙冠根折患者的有效率（98.00%）得到了顯著提升，和對照組（82.00%）相比，P<0.05。在并發癥方面，和對照組（14.00%）相比，觀察組（2.00%）更低，P<0.05。觀察組生活質量及各項功能評分較對照組更高，P<0.05。分析VAS評分，和對照組相比，觀察組評分更低，P<0.05。結論：在恒上前牙冠根折治療期間，通過予以牙體牙髓聯合正畸治療方法，改善患者咀嚼功能的同時有助于提高美觀舒適度，且降低了并發癥發生率，患者的疼痛感減少，生活質量有所提升，很大程度上提高了治療水平，在臨床中值得應用和推廣。

【Abstract】 Objective： To analyze and study the curative effect and effective rate of dental pulp combined with orthodontic treatment for crown and root fractures of permanent upper anterior teeth. Method： The study period was selected from January 2017 to January 2021. All 100 patients selected for this study were permanent upper anterior teeth crown and root fractures treated in our hospital， combined with the treatment of patients with permanent upper anterior teeth crown and root fractures. The difference of the method was divided into the control group and the observation group， 50 cases in each， routine treatment and dental pulp combined orthodontic treatment were carried out in sequence， and the treatment effect， complication rate and quality of life under the application of different intervention measures were compared. ， Various functions， VAS score. Result： The treatment effect was analyzed. After treatment， the effective rate （98.00%） of patients with permanent upper anterior tooth crown and root fracture in the observation group was significantly improved， compared with the control group （82.00%）， P<0.05. In terms of complications， compared with the control group （14.00%）， the observation group （2.00%） was lower， P<0.05. The quality of life and various function scores of the observation group were higher than those of the control group， P<0.05. Analyzing the VAS score， compared with the control group， the observation group had a lower score， P<0.05. Conclusion： During the treatment of permanent upper anterior teeth with crown and root fractures， the combination of dental pulp combined with orthodontic treatment can improve the patient's masticatory function and at the same time help improve aesthetics and comfort， and reduce the incidence of complications and patients pain The quality of life has been improved and the level of treatment has been improved to a large extent. It is worthy of application and promotion in clinical practice.

近年來，恒上前牙冠根折的患病率日趨升高，嚴重影響患者的身體健康在發病后，患者的牙冠根折至牙齦下3～4mm，需實施針對性的修復方法，從而改善患者的臨床癥狀。相關醫務人員去除冠部的斷端牙體組織，修復體和牙組織面無法完全密合，因此，需提高其重視程度，實施科學合理的正畸治療方法，從而提高患者的咬合功能和咀嚼功能。實施常規治療方法，雖然具有一定的效果，但是不符合患者的實際需求。實施牙體牙髓聯合正畸治療方法，可獲得理想的治療效果，且并發癥風險性較高，進一步提高了治療水平。本文擇取我院接收治療的恒上前牙冠根折患者（研究時段選取范圍2017年01月～2021年01月），觀察牙體牙髓聯合正畸治療的應用價值、治療效果、并發癥發生率、生活質量、各項功能、VAS評分，現有以下內容。

1 資料和方法

1.1 一般資料

研究時段選取范圍2017年01月～2021年01月，入選此次研究100例患者均為我院接收治療的恒上前牙冠根折，結合此間恒上前牙冠根折患者應用治療方法的差異，將其分別納入對照組和觀察組，各50例，依次實施常規治療及牙體牙髓聯合正畸治療，其中對照組：女18例，男32例，患者的年齡界限值為16-56歲，平均：（39.33±1.25）歲。觀察組：女19例，男31例，患者的年齡界限值為17-55歲，平均：（39.29±1.26）歲。兩組基礎資料相比P>0.05。

1.2 觀察指標

①分析治療效果，顯效：癥狀消失，有效：癥狀好轉，無效：癥狀無改善，有效率=顯效+有效。②分析并發癥發生率，包括：牙周炎、牙冠移位，密切觀察恒上前牙冠根折患者的并發癥發生情況，詳細記錄并實施統計學對比。

2 結果

2.1 分析治療效果

經治療后，觀察組恒上前牙冠根折患者的有效率（98.00%）得到了顯著提升，和對照組（82.00%）相比，P<0.05。

2.2 分析并發癥發生率

結果顯示，和對照組（14.00%）相比，觀察組（2.00%）更低，P<0.05。

2.3 分析生活質量

結果顯示，和對照組相比，觀察組評分更高，P<0.05。

2.4 分析各項功能

結果顯示，和對照組相比，觀察組更具優勢，P<0.05。

2.5 分析VAS評分

結果顯示，和對照組相比，觀察組評分更低，P<0.05。

3 討論

對于恒上前牙冠根折患者而言，承受著較大的生理痛苦和心理負擔。在實際的治療過程中，實施常規修復治療方法會侵犯生物學寬度，產生諸多的問題，比如：邊緣不密合、牙周組織破壞等，嚴重影響患者的治療效果。需要注意的是，恒上前牙冠根折實施常規修復影響其美觀度，產生脫落等并發癥的風險性較高。因此，在臨床的治療過程中，相關醫務人員應該將殘根拔除，然后實施修復義齒治療方法，從而提高整體治療效果。

研究結果顯示，分析治療效果，經治療后，觀察組恒上前牙冠根折患者的有效率（98.00%）得到了顯著提升，和對照組（82.00%）相比，P<0.05。實施牙體牙髓聯合正畸治療方法，協助患牙和牙周組織的結構比例，保證高度協調，有助于增加邊緣的密合性，從而提高了整體治療效果。實施牙體牙髓聯合正畸治療方法，延長牙齒的使用時間，患者的接受性較高，治療效果顯著。在并發癥方面，和對照組（14.00%）相比，觀察組（2.00%）更低，P<0.05。
