新型農村合作醫療多元化監管模式的探討與應用

[摘要] 江油市自開展新型農村合作醫療以來，始終堅持以“惠民、利民、便民”為出發點和落腳點，大膽管理，積極探索一條新型農村合作醫療的多元化監管模式，采取數據指標考核、制定目標責任協議書、實時系統醫療審核、電話回訪審核、抽查病歷審核、定期或不定期到定點醫療機構稽查審核相結合，確保了新型農村合作醫療基金在可控、可計劃的范圍內正常運行，規范了定點醫療機構的醫療服務行為，維護了參保人員的合法權益，使農民朋友真正得到了實惠。

[Abstract] Since the our city development of new rural cooperative medical care since，always adhere to the\"Huimin，benefit，convenience\"as the starting point and goal，bold management，actively explore a new rural cooperative medical care a plurality of supervision mode，take the data index assessment，goal setting responsibility agreement，real-time system of medical audit，audit，inspection records of telephone visit audit， regularly or irregularly to designated medical institutions inspection audit combination，to ensure that the new rural cooperative medical fund in the controllable， can plan the scope of normal operation， standardize the fixed medical institutions in medical service behavior， safeguard the legitimate rights and interests of the insured，so that farmers get the real benefit.

近年來，隨著江油市新型農村合作醫療（以下簡稱新農合）政策的大力宣傳，參保人員逐年遞增，國家、地方、財政資金的大量投入，2012年江油市參合農民61.7萬人，籌資基金近1.8億、參合率達99.7%[1]，為了保證基金平穩正常運行，江油市新型農村合作醫療中心不斷總結2007年3月實施新農合以來的工作經驗，完善服務運行管理體系，做到管理科學化、工作制度化、規范化，在原有監管模式的基礎上，積極探索適合當地的多種監管模式，取得了較好的成果，現報道如下。

1 實施多元化監管模式的方法

1.1 建立動態數據指標考核管理網絡、每月分析匯總[2]

由微機信息管理員每月對江油市60家各級定點醫療機構的住院人數、住院總費用、人均費用、藥品比例、自費藥品比例、報銷比例、審核扣減費用、補償金額、審批金額、基金使用量等進行分析匯總，設立了自定義公式，并制定了人均費用、藥品比例、報銷比例3個核心考核指標的控制費用，分別占每月補償款的60%、15%、25%，對當月指標超額的醫院一律從醫療墊付款中當月扣除。……
