某院外科Ⅰ類切口清潔手術預防用藥分析

[摘要] 目的 了解筆者所在醫院外科清潔手術患者預防應用抗菌藥物的情況，并探究其應用的合理性。 方法 對筆者所在醫院2009年下半年100例Ⅰ類切口清潔手術的預防用藥情況做分析。 結果 100例Ⅰ類切口清潔手術抗菌藥使用率100%，不合理用藥率達70%，不合理的情況主要體現在用藥檔次過高及術后用藥時間過長。 結論 筆者所在醫院外科Ⅰ類切口清潔手術還需進一步規范圍術期抗菌藥物的應用，以提高抗菌藥物合理應用水平。

Investigation and analysis of prophylactic application of antibiotics

in clean surgery patients of one hospital

[Abstract] Objective To investigate the prophylactic application of antibiotics in clean surgery patients of one hospital. Methods A retrospective drug utilization evaluation was performed for 100 cases of prophylactic application of antibiotics in clean surgery patients of the author's hospital in 2009. Results The utilization ratio of antibiotics in the clean surgery reached 100%.Unreasonable application rate was 70%. Unreasonable application was in overwork of the antibiotics and overlong of the time for prophylactic application. Conclusion The prophylactic application of antibiotics in clean surgery patients of the hospital should be improved further.

中國衛生部衛辦醫政發[2009]38號《衛生部辦公廳關于進一步加強抗菌藥物臨床應用管理的通知》，醫療機構應加強圍手術期抗菌藥物預防性應用的管理，改變過度依賴抗菌藥物預防手術感染的狀況，重點加強Ⅰ類切口手術預防使用抗菌藥物的管理和控制。現對2009年下半年本院外科Ⅰ類切口清潔手術的圍手術期預防用藥進行合理性評估，為提高筆者所在醫院合理用藥水平，規范圍手術期用藥提供參考。

1 資料與方法

1.1 一般資料

在筆者所在醫院病案室隨機抽取外科清潔手術病例，再經嚴格篩選，剔除高齡、糖尿病、免疫低下、其他部位感染、巳有細菌定植或感染等因素，得到100例確認是Ⅰ類切口清潔手術的病例，建立患者基本情況和用藥情況調查表，從抗菌藥使用率、用藥時機、用藥天數等方面對其進行統計分析。見表1。
