白內障超聲乳化術初學者并發癥的臨床分析

[摘要] 目的 探討分析白內障超聲乳化術初學者出現手術并發癥的原因。 方法 選取197例已行白內障超聲乳化術的白內障患者進行回顧性分析，對術中、術后發生的并發癥及其原因進行總結分析。 結果 所有患者術后視力均得到不同程度的提高，視力>0.3者：術后第1天131眼（66.50%），術后1周162眼（82.23%），術后1個月185眼（93.91%），術后3個月182眼（92.39%）。術中術后的并發癥主要有后囊膜破裂4眼（2.03%），角膜內皮水腫31眼（15.74%）等。 結論 初學者的白內障超聲乳化術療效、并發癥的發生率與術者的經驗、技術熟練程度密切相關，需要較長的學習曲線。

Clinical Analysis of Complications in Phacoemulsification by Beginner

[Abstract] Objective To discuss the causes of complications in phacoemulsification by beginner. Methods Phacoemulsification on 197 cataract patients was retrospectively analyzed， and the causes and prevention of Intraoperative and postoperative complications were studied. Results The postoperative visual acuity of all patients were improved to varying degrees. On postoperative day 1，week 1，month 1 and month 3 the proportion of patients who had acuity of 0.3 or better were 131 eyes（66.50%），162 eyes（82.23%），185 eyes（93.91%），182 eyes（92.39）. Intraoperative and postoperative complications were posterior capsular rupture （4 eyes，2.03%）， corneal endothelium edema（31 eyes，15.74%），et al. Conclusion The effect and complication rate of phacoemulsification are closely associated with surgical experience， technical proficiency， and it requires a long learning curve process.

白內障超聲乳化手術之所以在國內被廣泛應用是因為其具備手術切口小、組織損傷少、術后散光小、視覺質量好等優勢。但是白內障超聲乳化手術繁瑣、精細，學習過程長，對于初學者而言嚴重的并發癥很可能使其沒有自信心，最終導致掌握也很困難。白內障是造成我國視力殘疾的首位原因，占46.93%[1]，對于更多的眼科醫師掌握超聲乳化手術的需求也更加迫切，所以每一個初學者最關心的問題是如何安全渡過超聲乳化手術的初期階段，為探討分析白內障超聲乳化術初學者出現手術并發癥的原因，該研究者對2010年5月—2011年5月197例（197眼）白內障患者施行白內障超聲乳化聯合人工晶體植入術進行了總結，治療效果也相當顯著，但手術過程中也出現了一些問題，現報道如下。
