臨床藥師參與心臟機械瓣膜置換術后華法林抗凝治療管理的方法及作用分析

摘要：目的 本文就臨床藥師參與心臟機械瓣膜置換術后華法林抗凝治療管理的方法及作用進行分析與探討。方法 選擇我院2014年2月～2016年2月收治的行心臟機械瓣膜置換術患者66例，其后將本組患者按照來院先后順序平均分為兩組，每組患者33例。予以臨床藥師參與華法林抗凝治療管理的患者設為實驗組，未經臨床藥師參與的患者設為參照組，最后比對兩組患者的抗凝效果。結果 通過對兩組患者進行分析可知，實驗組患者的INR達標率和出院前INR達標率較比參照組明顯較高，INR首次達標時間較比參照組明顯較短，出血率同參照組進行比對明顯較低，組間統計后差異顯著。結論 患者行心臟機械瓣膜置換術時，臨床藥師參與抗凝管理，其效果理想，具有臨床應用價值。

Abstract：Objective To study the clinical pharmacists involved in mechanical heart valve replacement after the analysis and discussion of the method of warfarin anticoagulation management and effect.Methods 66 for mechanical heart valve replacement patients in our hospital in February 2014～ February 2016 were followed in this group of patients to the hospital in accordance with the order of the average divided into two groups，33 cases in each group be patient.Clinical pharmacists in warfarin anticoagulation management of patients for the experimental group，control group without the participation of clinical pharmacists in the patients，finally on anticoagulant effect of the two groups.Results Through the analysis of two groups of patients，patients in the experimental group the success rate of INR and INR before discharge compliance rate was significantly higher than the control group，INR for the first time，the standard time is significantly shorter than the control group，the bleeding rate compared with the reference group were significantly lower，the differences between the two groups after statistical significant.Conclusion When the patients underwent mechanical heart valve replacement，the clinical pharmacists participated in anticoagulation management，and the effect was ideal，and it has clinical value.

在心臟病疾病中，最為常見的為風濕性心臟病，該疾病會對心臟瓣膜造成影響，同時致使瓣膜功能出現異常[1]。從目前的治療方法來看，臨床常采用瓣膜修復法和人工瓣膜置換手術，雖然上述治療手段可以使患者的心功能得以改善，但是患者手術后較易出現血栓，因此，對于該類患者需要予以抗凝治療。從抗凝藥物來看，香豆素應用較為廣泛，近年來，華法林抗凝廣泛應用于臨床[2]。為此，本次研究為探究臨床藥師參與華法林抗凝的臨床效果，選擇我院2014年2月～2016年2月收治的行心臟機械瓣膜置換術患者66例，現將研究結果進行如下敘述?！?br>
