集束化護理在行連續性腎臟替代治療的老年危重癥患者中的應用效果

[摘要]目的 探討集束化護理在行連續性腎臟替代治療的老年危重癥患者中的應用效果。方法 選取2017年1～12月在我院行連續性腎臟替代治療的80例老年危重癥患者作為研究對象，根據隨機數字表法分為常規護理組和集束化護理組，每組各40例。比較兩組患者治療過程中常見并發癥（壓瘡、感染、深靜脈血栓和水電解質紊亂）的發生情況以及患者的治療依從性。結果 集束化護理組患者治療過程中并發癥（壓瘡、感染、深靜脈血栓和水電解質紊亂）的發生率均低于常規護理組（P<0.05）；集束化護理組患者的治療總依從率（87.50%）顯著高于常規護理組（55.00%）（P<0.05）。結論 對行連續性腎臟替代治療治療的老年危重癥患者而言，集束化護理能顯著改善治療過程中的并發癥，提高患者的治療依從性，值得推廣。

[Abstract] Objective To explore the application effect of cluster nursing in old critically ill patients with continuous renal replacement therapy. Methods From January to December 2017， A total of 80 old critically ill patients treated with continuous renal replacement therapy in our hospital were selected as the subjects， and divided into common nursing group and cluster nursing group according to the random number table method， with 40 cases in each group. The complication （pressure sores， infection， deep venous thrombosis and disturbance of water and electrolyte） and treatment compliance during the treatment in the two group were compared. Results The occurance of pressure sores， infection， deep venous thrombosis and disturbance of water and electrolyte in cluster nursing group were lower than those in common nursing group （P<0.05）. The total compliance rate of treatment in cluster nursing group was 87.50%， significantly higher than that in the common nursing group （55.00%） （P<0.05）. Conclusion Cluster nursing can decrease the complication and increase the treatment compliance of continuous renal replacement therapy in old critically ill patients， it is worth promoting clinically.

連續性腎臟替代治療（continuous renal replacement therapy，CRRT）作為如今重癥監護室常用的一種血液凈化方式，對危重癥患者起著有效的臟器功能支持，在重癥患者生命維持中凸顯重要價值。老年危重患者的身體素質往往較差，長期治療會導致嚴重的治療并發癥[1]，且由于危重病情常使患者身心俱創，治療依從性往往不理想，因此對老年危重患者而言，該治療方式對護理要求十分高。然而，當前的常規護理方法存在很多缺陷，往往不能對行連續性腎臟替代治療的老年危重患者起到有效的護理效果。研究顯示，集束化護理依據循證醫學而實施，具有全面、連續、個體化等優點，已在多個領域良好應用[2-3]。本文旨在探討集束化護理在行連續性腎臟替代治療的老年危重癥患者中的應用效果，現報道如下。……
