[摘要] 目的 比較超聲彈性成像(UE)和高頻彩色多普勒超聲(HFCDU)對乳腺癌的診斷價值。方法 方便選取2018年1月—2019年6月因乳腺占位病變而就診的40例患者,依次實行HFCDU檢查、UE檢查。比較兩種方法的準確率、誤診率、特異性、敏感性、陽性預測值和陰性預測值。 結果 UE的準確率為92.00%,優于HFCDU的72.00%,其誤診率為8.00%,低于HFCDU的28.00%,差異有統計學意義(χ2=6.775,P=0.009<0.05)。UE的特異性、敏感性分別為90.00%和85.00%,優于HFCDU的70.00%和60.00%,差異有統計學意義(χ2=5.000、6.270,P=0.025、0.012<0.05)。UE的陽性預測值和陰性預測值分別為86.00%和90.00%,優于HFCDU的32.00%和36.00%,差異有統計學意義(χ2=30.136、29.340,P=0.000、0.000<0.05)。結論? UE診斷乳腺占位病變性質比HFCDU價值更高,其不僅具有更加優秀的準確性及敏感性、特異性,其陰性、陽性預測值也相對更高,可以作為乳腺癌的有效評估方法在臨床中積極運用。

[Abstract] Objective To compare the diagnostic value of ultrasound elastography (UE) and high-frequency color Doppler ultrasound (HFCDU) for breast cancer. Methods A total of 40 patients who were diagnosed with breast mass lesions from January 2018 to June 2019 were convenient selected, and HFCDU and UE examinations were performed in this order. The accuracy, misdiagnosis rate, specificity, sensitivity, positive predictive value and negative predictive value of the two methods were compared. Results The accuracy rate of UE was 92.00%, which was better than 72.00% of HFCDU. Its misdiagnosis rate was 8.00%, which was lower than 28.00% of HFCDU. The difference was statistically significant(χ2=6.775, P=0.009<0.05). The specificity and sensitivity of the UE were 90.00% and 85.00%, respectively, which were better than 70.00% and 60.00% of the HFCDU. The differences were statistically significant (χ2=5.000, 6.270, P=0.025, 0.012<0.05). The positive and negative predictive values of UE were 86.00% and 90.00%, respectively, which were better than 32.00% and 36.00% of HFCDU, and the differences were statistically significant (χ2=30.136,29.340, P=0.000, 0.000<0.05). Conclusion The diagnosis of breast mass lesions by UE is more valuable than HFCDU. It not only has better accuracy, sensitivity and specificity, but also has relatively higher negative and positive predictive values. It can be used as an effective assessment method for breast cancer in clinical practice. Active use.

乳腺癌是近些年來危害女性健康一種常見惡性腫瘤,其不僅發病率高,甚至不斷呈現出發病患者年輕化的趨勢,對于乳腺癌患者而言,手術是最佳治療方式,其中保乳手術是最常見的一種手術方式,它較傳統手術方式創傷小,且可以保留乳房,但癌組織已經擴散或發生轉移的中晚期腫瘤患者卻不適合此種治療,只能通過比如放化療等方法來延續生命,所以想要改善乳腺癌患者預后,提升其生活質量甚至生存率,給予患者盡早的準確診斷與有效的治療是非常必要的[1]。目前用于診斷乳腺癌的超聲技術有許多,如何選擇最適合的超聲方法讓患者最大受益,需要臨床謹慎選擇。該文方便選取2018年1月—2019年6月因乳腺占位病變而就診的40例患者,試分析超聲彈性成像(UE)、高頻彩色多普勒超聲(HFCDU)這兩種超聲技術對乳腺癌的評價效果,現報道如下。
