患者出院準備度的研究現狀

摘要：出院準備度是對患者及家屬離開急性照護的評估和預判，能夠了解患者是否做好出院準備。出院準備度的評估內容不僅包括患者的生理穩定性，還包括患者的心理因素及社會方面因素，良好的住院準備對患者的安全、疾病康復與轉歸具有重要作用。本文就出院準備度的概念、評估工具、影響因素及干預措施作一綜述，以期為提高患者的出院準備度提供參考依據，更好地促進患者康復。

Current Status of Research on Patient Discharge Readiness

Abstract：Discharge readiness is the assessment and prediction of patients and their families leaving acute care， and it can understand whether the patient is ready for discharge. The content of the assessment of readiness for discharge includes not only the physiological stability of the patient， but also the psychological and social factors of the patient. Good preparation for hospitalization plays an important role in the safety， rehabilitation and outcome of the patient. This article reviews the concepts， assessment tools， influencing factors and intervention measures of hospital discharge readiness， in order to provide a reference for improving patients' discharge readiness and better promote patient recovery.

隨著快速康復外科理念的普及，患者的平均住院日較以前縮短[1]，然而平均住院日的縮短意味著患者出院時身體各項機能可能尚未完全恢復[2]。出院準備度能夠評價患者離開醫院后進一步康復的能力，能夠預測患者能否安全渡過出院后的過渡期[3]，對患者出院后的治療效果、生活質量、轉歸及再次入院至關重要[4]。本文就出院準備度的概念、評價工具、影響因素及提高患者出院準備度的護理措施作一綜述，以期為臨床醫護工作者提高患者的出院準備度提供參考依據。

1出院準備度的概念

出院準備度的概念是1979年率先由英國保育協會學者Fenwick提出，指醫務人員綜合病人的心理、生理和社會方面的健康狀況，分析患者在多大程度上具備離開醫院、回歸社會、進一步康復的能力[5]。Galvin EC等[6]通過文獻回顧提取了"出院準備"的以下屬性：生命體征穩定、足夠的支持、心理能力和足夠的信息和知識。……
