方法 計算機檢索數據庫包括Pubmed?Web of Science?COCHRANE LIBRAR

Y?中國生物醫學數據庫?萬方數據庫和維普數據庫，檢索時限從建庫至2013年6月，同時追

索納入文章的參考文獻?嚴格按照納入標準篩選文獻?提取資料和評價質量后，應用Review

Manager 5.2軟件對所納入的研究結果進行Meta分析?

結果 共納入11篇RCT文獻資料，包括662例心力衰竭伴低T3綜合征患者?M

eta分析結果顯示：與常規治療組相比，甲狀腺素組（甲狀腺激素加常規治療）的左心室射

血分數（LVEF）和心輸出量（CO）明顯增加（MD=5.16，95%CI=3.11～7.21，P<0.01

；MD=0.55，95%CI=0.33～0.77，P<0.01），評價心室舒縮功能的E/A值升高和左心室

舒張末內徑（LVDD）降低（MD=0.22，95%CI=0.09～0.34，P<0.01；MD=-4.14，95%CI=

-6.66～-1.61，P<0.01）?

結論 心力衰竭伴低T3綜合征患者在常規治療基礎上加用甲狀腺激素可明

顯提高患者的心臟射血功能及改善心室的舒縮功能?

[Abstract] Objective To systematically assess the curative eff

ect of thyroxine in patients with heart failure complicating low T3 syndrome.

Methods Retrieving

databases including Pubmed， Web of Science， Cochrane Library， CBM， WanFang datab

ase and VIP database. The retrieval time limited from the foundation of databas

e to June 2013. Meantime， references of articles were searched. According to t

he inclusion criterion， relevant articles were screened. After extracting data，

quality of data was assessed， and meta_analysis was performed by RevMan 5.2.

Results A total of 11 RCTs involving 662 patients were included

. The result of meta_an

alyses showed that， compared with the control group， after short_term treatment

with thyroxine， left ventricular ejection fraction （LVEF） and cardiac output （C

O） of the treatment group obviously increased （MD=5.16， 95%CI=3.11～7.21， P

<0.01； MD=0.55， 95%CI=0.33～0.7 P<0.01）. Besides， the evaluation ind

ex of ventricular diastolic function E/A was improved and left ventricular end d

iastolic diameter （LVDD） was decreased （MD=0.22， 95%CI=0.09～0.34，P<0.01

； MD=-4.14， 95%CI=-6.66～-1.61，P<0.01）.

Conclusions Thyroxine based on routine treatment can improve

the heart ejection function and the ventricular diastolic function for patients

with heart failure complicating low T3 syndrome.endprint

心力衰竭是各種心臟疾病導致心功能不全的最終結局，心肌收縮力下降使得心排血量不能滿

足機體代謝的需要，同時伴有一系列神經內分泌及代謝功能異常，其中甲狀腺激素的變化近

年來倍受關注?低T3綜合征（又名亞臨床甲狀腺功能減退?正常甲狀腺功能病態綜合征）是

指非甲狀腺疾病所致的甲狀腺功能異常?相關調查顯示約13%～30%的心衰患者出現甲狀腺功

能異常?心力衰竭時甲狀腺激素的變化常以單純低T3綜合征為主，其次還可以表現為低T4綜
