臨床路徑在單病種付費中的應用

摘 要：傳統的按醫療服務項目收費方式存在醫療費用較高、服務項目價格難以確定等問題。近些年，研究表明單病種付費方式能夠一定程度上控制醫療費用的增長，提高醫療資源利用率和醫療服務質量，能夠有效地控制醫療成本，縮短平均住院時間。其中，臨床路徑能夠保證單病種付費方式的合理性和科學性，并能規范醫療行為，同時對單病種付費方式的定義和優缺點、臨床路徑的定義及其在單病種付費方式中的應用進行闡述。

Abstract：Traditional methods of charging for medical service items have problems such as high medical expenses and difficulty in determining the price of service items.In recent years，studies have shown that single disease payment methods can control the growth of medical expenses to a certain extent，improve the utilization of medical resources and the quality of medical services，and can effectively control medical costs and shorten the average hospital stay.Among them，the clinical pathway can guarantee the rationality and scientificity of the single disease payment method，and can standardize the medical behavior.At the same time，the definition， advantages and disadvantages of the single disease payment method，the definition of the clinical path and the payment method in the single disease type. The application is elaborated.

隨著我國醫療衛生事業不斷地發展，醫藥衛生體制改革，增強了衛生事業的公益性，保障公民的基本醫療衛生需求，但是仍存在“看病難，看病貴”的問題。當前，我國醫療服務付費方式仍然是以服務項目付費為主，按服務項目付費雖然明確了收費標準，但仍有一些不足，首先該付費方式會增加醫療服務次數，無法確定醫療費用，其次對每個服務項目的價格缺乏科學的確定方式，最后容易傾向于發展高、精、尖醫學科學技術，忽視常見病、慢性病、多發病的防治工作[1]。為此，各衛生醫療單位都在積極地尋求有效的解決方案。其中單病種付費方式的出現，為控制醫療成本帶來了新的希望。單病種付費能夠合理控制部分疾病的診療成本，提高工作效率，有效利用醫療資源，縮短平均住院時間，提高患者滿意度，促進醫院的可持續性發展。……
