血府逐瘀湯加味而成的中藥湯劑聯合奧氮平治療腦外傷致精神障礙的臨床效果

【摘要】 目的：探討對腦外傷導致精神障礙患者行中西醫聯合治療的臨床效果。方法：選取2014年12月-2016年12月筆者所在醫院收治的48例腦外傷致精神障礙患者作為研究對象，根據數字隨機表法進行分組研究，單數為聯合組，雙數為單一組，各24例。聯合組行加味血府逐瘀湯+奧氮平聯合治療，單一組僅行奧氮平治療，觀察并比較兩組治療有效率、不良反應發生率、陽性和陰性癥狀量表（PANSS）評分及副反應癥狀量表（TESS）評分的變化情況。結果：聯合組總體有效率91.67%，顯著高于單一組的66.67%，聯合組不良反應發生率8.33%，顯著低于單一組的33.33%，兩組治療前的PANSS評分、TESS評分比較差異無統計學意義（P>0.05），治療后聯合組PANSS評分、TESS評分均優于單一組，差異有統計學意義（P<0.05）。結論：對腦外傷導致精神障礙患者行加味血府逐瘀湯+奧氮平中西醫聯合治療，能夠達到治標治本的效果，極大程度緩解患者精神障礙等不良癥狀，在一定程度上減少不良反應的發生。

【Abstract】 Objective：To investigate the clinical effect of combined treatment of traditional Chinese and Western medicine on patients with mental disorders caused by brain trauma.Method：From December 2014 to December 2016，48 patients with traumatic brain injury with mental disorders in our hospital were selected as the research object，studies were grouped according to the random number table method，singular for the combined group，even as the single group，24 cases in each.The combined group was treated with Xuefu Zhuyu Decoction plus Olanzapine，the single group only received Olanzapine treatment，the two groups were observed and compared the effective rate，adverse reaction rate，positive and negative symptom scale（PANSS） score and emergent symptom scale（TESS） scores changed.Result：The combined group overall efficiency was 91.67%，it was significantly higher than 66.67% of the single group，the adverse reaction probability in the combined group was 8.33%，it was significantly lower than 33.33% of the single group，there were no significant differences between the two groups before treatment PANSS score，TESS score（P>0.05），after treatment，the combined group PANSS score and TESS score were better than the single group，the differences were statistically significant（P<0.05）.Conclusion：The patients of traumatic brain injury with mental disorders treat with Olanzapine plus traditional Chinese medicine，can achieve temporary cure effect，greatly alleviate the patients with mental disorders and other adverse symptoms，reduce the incidence of adverse reactions in a certain extent.

經過大量研究表明，腦外傷患者在患病初期階段將會出現不同程度的精神障礙反應，現代醫學通常采取西藥治療、降顱壓治療等方式，但是治療效果并不顯著[1-2]?，F代臨床醫學上通常采用西藥抗炎治療和降顱壓治療，但是容易增加患者不良反應的發生率，若未給予及時治療和護理，將會嚴重影響治療效果。本研究將筆者所在醫院2014年12月-2016年12月筆者所在醫院收治的48例腦外傷致精神障礙患者作為研究對象，分別給予單一西藥治療和中西醫結合治療，觀察兩組治療效果。

1 資料與方法
