【摘要】 目的 分析C反應蛋白和白細胞(WBC)聯合檢測在兒科細菌性感染疾病診斷中的應用。方法 120例患兒, 將細菌性感染的60例患兒設為分析組, 無細菌性感染的60例患兒設為對照組。對兩組患兒進行C反應蛋白和WBC檢測, 比較兩組患兒的C反應蛋白和WBC陽性情況。結果 分析組患兒的C反應蛋白陽性例數為49例, 陽性率為81.67%;陰性例數為11例, 陰性率為18.33%;對照組患兒的C反應蛋白陽性例數為8例, 陽性率為13.33%;陰性例數為52例, 陰性率為86.67%;分析組患兒的C反應蛋白陽性率高于對照組, 差異有統計學意義(P<0.05)。分析組患兒的WBC陽性例數為34例, 陽性率為56.67%;陰性例數為26例, 陰性率為43.33%;對照組患兒的WBC陽性例數為4例, 陽性率為6.67%;陰性例數為56例, 陰性率為93.33%;分析組患兒的WBC陽性率高于對照組, 差異有統計學意義(P<0.05)。分析組患兒的C反應蛋白陽性率高于WBC, 差異有統計學意義(P<0.05)。結論 C反應蛋白和WBC聯合檢測在兒科細菌性感染疾病診斷中具有重要應用意義, 是體現早期診斷和監測治療的重要指標, 為抗生素的正確使用和預后做鋪墊。

【Abstract】 Objective To analyze the application of combined detection of C reactive protein and white blood cell (WBC) in the diagnosis of pediatric bacterial infection. Methods There were 60 children with bacterial infection as analysis group, and 60 children without bacterial infection as control group. The C reaction protein and WBC were detected in two groups, and the positive conditions of C reactive protein and WBC in the two groups were compared. Results The analysis group had 19 C reactive protein positive cases, with positive rate as 81.67%, 11 negative cases, with negative rate as 18.33%, while the control group had 8 C reactive protein positive cases, with positive rate as 13.33%, 52 negative cases, with negative rate as 86.67%. The analysis group had higher C reactive protein positive rate than the control group, and the difference was statistically significant (P<0.05). The analysis group had 34 WBC positive cases, with positive rate as 56.67%, 26 negative cases, with negative rate as 43.33%, while the control group had 4 WBC positive cases, with positive rate as 6.67%,
56 negative cases, with negative rate as 93.33%. The analysis group had higher WBC positive rate than the control group, and the difference was statistically significant (P<0.05). The C reactive protein positive rate in analysis group was higher than WBC positive rate, and the difference was statistically significant (P<0.05).
Conclusion Combined detection of C reactive protein and WBC has important application significance in the diagnosis of pediatric bacterial infectious diseases. It is an important index reflecting early diagnosis and monitoring treatment, and pave the way for the correct use and prognosis of antibiotics.

C反應蛋白作為急性時相反應蛋白之一, 具有體現炎性反應的作用, 這種反應蛋白是機體在應激情況下產生的, 通過肝臟合成[1]。同時, C反應蛋白也是體現組織壞死急性期的一種炎癥敏感標志物, 在臨床上比較多見。健康人體一般每天合成1~10 mg C反應蛋白, 而在急性期炎性反應的條件下每天可合成高達1 g的C反應蛋白[2]。針對兒科不同類型的細菌性感染疾病而言, 早期C反應蛋白會大幅度上升, 且發生在WBC上升前[3]。有大量文獻報告證明, 將C反應蛋白和WBC聯合檢測對臨床診斷有重要意義[4]。本院分析了C反應蛋白和WBC聯合檢測在兒科細菌性感染疾病診斷中的應用, 詳情報告如下。endprint
