[摘要]"術后譫妄(postoperative"delirium,POD)是老年患者在麻醉及手術后出現的一種常見的神經精神類并發(fā)癥,主要表現為注意力、意識和認知功能急性障礙。在老年人群中,POD的發(fā)生率為16%~73%,該發(fā)生率受手術類型、圍手術期管理策略及其他多種因素影響。POD的發(fā)生是多種易感因素和誘發(fā)因素相互作用的結果,其與重癥監(jiān)護病房住院時間延長、醫(yī)療成本增加、死亡率升高及術后認知功能障礙和日常生活能力下降緊密相關。本文綜述近年來老年患者POD的危險因素,旨在為臨床實踐提供理論支持,幫助醫(yī)護人員識別高風險患者并實施有針對性的干預措施,以降低POD發(fā)生率,改善患者預后。

術后譫妄(postoperative"delirium,POD)作為一種常見的急性認知障礙,日益受到學者關注。POD主要表現為注意力不集中、意識混亂和短期記憶力減退[1]。大型手術患者的POD發(fā)生率高達73%[2]。POD不僅延長患者住院時間,增加醫(yī)療成本,還可造成患者長期認知損害,提高患者的死亡率[3]。深入研究POD的影響因素對提升臨床護理質量、減少患者并發(fā)癥、改善患者預后具有重大意義。本文系統梳理影響老年患者POD的各種危險因素,以期為臨床實踐提供有力支撐,降低POD發(fā)生率,改善患者預后。
1""POD的易感因素
1.1""年齡
研究表明高齡是POD的重要危險因素之一[4]。POD的發(fā)生風險隨著年齡的增加而升高。研究顯示75歲及以上老年患者的POD發(fā)生率是年齡lt;75歲患者的2.54倍[5]。Zhu等[6]研究發(fā)現,隨著年齡的增長,大腦的退行性變化導致個體對譫妄的易感性增強;在相同的誘發(fā)因素下,高齡患者發(fā)生譫妄的概率顯著增加。在衰老過程中,大腦經歷連通性減弱、神經炎癥增加、神經膠質細胞功能改變及腦血管功能障礙等變化[4]。這些生理變化使大腦更易受到能量和氧氣供應中斷及循環(huán)中炎癥因子的影響,從而增加POD的風險。
1.2""衰弱狀態(tài)
衰弱綜合征通常被定義為一種由機體退行性改變和多種慢性疾病引起的機體易損性增加綜合征[7]。衰弱綜合征的臨床表現主要為身體功能衰退,易受外界壓力因素影響,進而導致健康狀況急劇惡化。研究表明衰弱狀態(tài)與多種不良健康結局相關,包括POD[8]。低骨骼肌質量作為衰弱狀態(tài)的客觀評價指標,近年來受到學者們的廣泛關注。一項Meta分析結果顯示低骨骼肌質量與POD發(fā)生密切相關[9]。提示機體整體功能下降、衰弱狀態(tài)可導致肌肉質量和認知功能下降,進而增加POD的風險[9]。
1.3""腦血管事件
患者術前患有顱外頸動脈和椎動脈疾病、顱內動脈病變等腦血管疾病可顯著增加POD風險;高血壓、心房顫動和腦卒中病史等腦血管事件的風險因素亦與POD風險增加顯著相關[10]。在老年手術患者中,盡管明顯的術后腦卒中較罕見,但影像學檢查發(fā)現患者的無癥狀缺血性腦損傷發(fā)生率為7%~10%,這與POD風險增加超過2倍顯著相關[11]。一項針對肺移植老年患者的小型隊列研究發(fā)現,腦灌注壓每下降10mmHg(1mmHg="0.133kPa),POD風險顯著增加2.08倍[12]。這些研究結果表明術前細致評估患者的腦血管狀態(tài)對降低POD風險至關重要。
1.4""維生素D缺乏
維生素D是一種脂溶性類固醇激素,其受體廣泛分布于大腦的各個區(qū)域,如皮層、海馬體和下丘腦,這些區(qū)域對認知功能至關重要。研究表明維生素D缺乏與術后認知功能障礙和POD風險增加相關[13]。維生素D的缺乏可削弱大腦的神經保護和免疫調節(jié)功能[13]。研究證實維生素D通過調節(jié)大腦中的乙酰膽堿和多巴胺等關鍵神經遞質發(fā)揮其神經保護作用[14]。最新研究通過非線性孟德爾隨機化分析證實,維生素D的免疫調節(jié)作用可通過降低C反應蛋白水平、減少神經炎癥進而在POD預防中發(fā)揮作用[15]。盡管如此,維生素D補充劑在POD預防中的確切效果及其作用機制仍需通過嚴格的臨床試驗予以進一步闡明。
1.5""術前情緒狀態(tài)與睡眠障礙
焦慮和抑郁是手術患者術前的常見情緒狀態(tài),這與患者擔憂手術結果和術后恢復有關。研究表明術前焦慮與POD獨立相關[16];術前抑郁與心臟手術后POD的發(fā)生風險呈正相關[17]。因此,患者術前情緒狀態(tài)的評估與干預至關重要。此外,睡眠障礙也被證實與POD風險增加相關[18]。He等[19]通過Meta分析發(fā)現患者手術前后的睡眠障礙均與POD風險顯著增加有關。這種關聯普遍存在于各類患者中,可能涉及神經、內分泌功能改變,炎癥反應增加及認知功能受損等機制。值得注意的是,術前情緒狀態(tài)與睡眠障礙之間存在顯著的相互影響。焦慮和抑郁可降低睡眠質量,睡眠障礙可加劇情緒問題,形成惡性循環(huán)。阻塞性睡眠呼吸暫停作為一種常見的睡眠障礙已被研究證實與POD發(fā)生顯著相關[20]。盡管相關高質量研究資料有限且結果異質性較大,但現有證據支持阻塞性睡眠呼吸暫停與POD存在相關性。鑒于此,未來的研究應致力于闡明各類睡眠障礙與POD的確切關系,并評估針對性治療措施對改善患者術后恢復的潛在效益。
2""POD的誘發(fā)因素
2.1""腎功能不全與電解質失衡
術前腎功能不全及電解質失衡是老年骨科手術患者發(fā)生的POD重要危險因素[21]。電解質失衡患者的POD風險是電解質正常患者的2.864倍[21]。術后肌酐水平≥68.20μmol/L的患者發(fā)生POD的風險可增加2.66倍[21]。Davani等[22]觀察652例≥65歲且無術前譫妄的髖關節(jié)骨折患者,術中接受相同劑量的阿片類藥物治療,結果顯示慢性腎臟病(chronic"kidney"disease,CKD)患者術后第2天的POD發(fā)生風險顯著升高,而非CKD患者則未顯示出該相關性。其機制可能涉及腎清除率下降導致的神經毒素積累、全身性細胞因子介導的神經炎癥增強及容量過載狀態(tài)。術前評估患者的腎功能和電解質狀況對識別POD風險至關重要。
2.2""手術時長
在實際臨床工作中,手術持續(xù)時間被認為是POD的另一重要風險因素。一項回顧性隊列研究證實長時間頭頸部手術與POD之間的相關性。該研究納入170例年齡gt;60歲且接受5h以上頭頸部手術的患者,結果顯示40例患者發(fā)展為POD,手術時長與POD的發(fā)生具有顯著相關性[23]。長時間手術增加患者遭受的應激程度,包括但不限于血流動力學不穩(wěn)定、氧化應激、炎癥反應增強和代謝需求變化等,這些因素均會增加POD風險。
2.3""術中低血壓
術中低血壓在手術過程中較為常見,其可導致重要器官氧供減少,進而引起心、腎和腦損傷[24]。研究顯示持續(xù)低血壓(≥5min)與POD顯著相關[25]。然而也有研究認為術中低血壓與術后認知功能障礙或POD之間的關聯并不明確[26]。盡管研究結果并不一致,但術中低血壓可減少腦灌注,影響腦部穩(wěn)態(tài),進而引發(fā)POD。老年患者在全身麻醉時,由于動脈彈性減弱和其他共病,應對缺血和缺氧的能力下降,可能更易受到影響。為此,實施有效的血壓管理策略對預防POD具有重要意義。
2.4""麻醉鎮(zhèn)靜深度
研究指出深度麻醉可減少早期術后疼痛,但可能增加POD風險[27]。一項隨機對照試驗顯示深度麻醉組的POD發(fā)生率為28%,輕度麻醉組為19%,提示深度麻醉與腦電活動的過度抑制有關[28]。暴發(fā)抑制是一種在深度麻醉狀態(tài)下觀察到的異常腦電現象。近年來,暴發(fā)抑制占空比被用于量化這一現象。一項研究發(fā)現較高的暴發(fā)抑制占空比與POD顯著相關,提示深度麻醉可能是POD的危險因素[29]。然而也有研究表明麻醉深度較淺也可能增加POD風險,因其可能導致患者體驗疼痛和應激反應,進而影響認知功能和情緒調節(jié)[30]。因此,維持適當的麻醉鎮(zhèn)靜深度對預防POD至關重要。
2.5""苯二氮類藥物的使用
膽堿能缺乏、多巴胺過量、神經遞質γ-氨基丁酸的抑制作用均對POD的發(fā)展有影響。臨床證據表明POD發(fā)生率與所用鎮(zhèn)靜劑的種類和使用方式相關[31]。作為一種常用的鎮(zhèn)靜劑,苯二氮?類藥物通常用于術前及麻醉過程中。然而,苯二氮?類藥物與POD之間的關系仍存在爭議。早期研究認為老年患者圍手術期使用苯二氮?類藥物可能是POD的危險因素之一[32],但近期研究表明苯二氮?類藥物并未增加POD的發(fā)生率[33]。歐洲麻醉與重癥監(jiān)護學會相關指南不推薦在老年和危重患者中使用苯二氮?類藥物預防和治療POD[34]。
3""小結
本文系統分析近年來老年患者POD的危險因素,并強調臨床實踐中多模式評估和干預的必要性。研究指出年齡、衰弱狀態(tài)、腦血管問題、維生素D缺乏、情緒和睡眠問題、手術及麻醉管理等均與POD發(fā)生風險緊密相關。鑒于此,建議在臨床實踐中整合POD篩查與預防,包括認知和情緒評估、腎功能監(jiān)測、手術流程控制和麻醉技術優(yōu)化。同時,鑒于苯二氮?類藥物的爭議性,提倡謹慎使用并遵循個體化治療原則。
